乳腺癌术后患者化疗期间心理弹性与生活质量的关系及其影响因素分析
Analysis the relationship between psychological resilience and quality of life and its influencing factors in breast cancer patients during postoperative chemotherapy
【摘要】 目的探讨乳腺癌术后患者化疗期间心理弹性水平与生活质量的关系及其影响因素。方法选取128例乳腺癌术后接受化疗的患者,采用Connor-Davidson心理弹性量表(CD-RISC)、生活质量测定量表(FACTB)对所有患者化疗期间的心理弹性和生活质量进行评价,分析乳腺癌术后患者化疗期间心理弹性的影响因素,并对乳腺癌术后患者化疗期间心理弹性与生活质量的相关性进行分析。结果乳腺癌术后患者化疗期间的CD-RISC量表总评分为(61.46±13.11)分,FACT-B量表总评分为(70.59±14.86)分。不同受教育程度、肿瘤分期、付费方式、化疗疗程乳腺癌术后患者化疗期间的心理弹性评分比较,差异均有统计学意义(P﹤0.05);其中受教育程度高、肿瘤分期低、有医保、化疗疗程短的乳腺癌术后患者化疗期间的心理弹性水平高。乳腺癌术后患者化疗期间CD-RISC量表的总评分及自强、坚韧、乐观维度评分与FACT-B量表的总评分、生理状况、社会/家庭状况、情感状况、功能状况、附加关注状况评分均呈正相关(P﹤0.05)。结论乳腺癌术后患者化疗期间的心理弹性水平较低,而受教育程度、肿瘤分期、付费方式及化疗疗程均可能影响乳腺癌术后患者化疗期间的心理弹性水平,且患者化疗期间心理弹性水平与生活质量呈正相关。加强乳腺癌术后患者化疗期间的心理弹性水平,有助于提高患者的生活质量。
【Abstract】 Objective To explore the relationship between the psychological resilience and quality of life in breast cancer patients during postoperative chemotherapy, and assess the related factors. Method A total of 128 breast cancer patients who were administered with postoperative chemotherapy were enrolled in the study. The Connor-Davidson resilience scale(CD-RISC) and the functiond assessment of cancer therapy-breast(FACT-B) were used to evaluate the psychological resilience and quality of life of those patients during postoperative chemotherapy, and factors influencing their psychological resilience were further investigated, additionally, the correlation between patients’ psychological resilience and quality of life was assessed. Result The total score of CD-RISC in breast cancer patients during postoperative chemotherapy was(61.46±13.11) points, and that by FACT-B was(70.59±14.86) points. There were statistically significant differences in the psychological resilience score of breast cancer patients during postoperative chemotherapy with different education background, tumor stages, payment options and chemotherapy courses(P<0.05); in addition, patients who had higher education level, lower tumor stage, with medical insurance and shorter chemotherapy courses may have higher psychological resilience level. Among breast cancer patients, the overall score, self-reliance, tenacity, optimism scores of CD-RISC were positively correlated with the overall score, physical well-being, social/family well-being, emotional wellbeing, functional well-being and additional concerns of FACT-B scale(P<0.05). Conclusion Breast cancer patients receiving postoperative chemotherapy have generally lower psychological resilience level. The education level, tumor stages, payment options and chemotherapy courses were factors that influence the breast cancer patients’ psychological resilience, which is positively correlated with patients’ quality of life. Strengthening the patient’s level of psychological resilience can help improve the quality of life of patients.
