高危型HPV感染与宫颈癌前病变及宫颈癌的关系

【摘要】 目的探讨高危型人乳头状瘤病毒(HPV)感染与宫颈癌前病变及宫颈癌的关系。方法选取149例宫颈癌前病变患者和51例宫颈癌患者,分别作为癌前病变组和宫颈癌组。比较两组患者高危型HPV感染检出率、HPV相对表达量及病毒载量,并检测高危型HPV感染患者的亚型分布情况。结果共检出高危型HPV感染164例,其中癌前病变组高危型HPV检出率为77.85%,明显低于宫颈癌组患者的94.12%,差异有统计学意义(P﹤0.01)。癌前病变组患者HPV相对表达量和病毒载量,均明显低于宫颈癌组患者,差异均有统计学意义(P﹤0.01)。高危型HPV亚型以HPV16、HPV18为主,分别占34.75%、20.73%。癌前病变组患者不同高危型HPV亚型阳性率均明显低于宫颈癌组患者,差异均有统计学意义(P﹤0.01)。结论宫颈癌前病变及宫颈癌与高危型HPV感染密切相关,高危型HPV持续感染是宫颈癌前病变及宫颈癌主要致病因素,因此,早期检测HPV感染情况对预防宫颈癌前病变及宫颈癌具有重要意义。

【Abstract】 Objective To investigate the relationship between high-risk human papillomavirus(HPV) infection and cervical precancerous lesions and cervical cancer. Method The research included 149 cases with cervical precancerous lesions and 51 cases of cervical cancer, who were assigned as precancerous lesions group and cervical cancer group, respectively. The incidence of high-risk HPV infection, relative expression of HPV and viral load were compared between the two groups. The distribution of subtypes of high-risk HPV infection was detected. Result A total of 164 cases of high-risk HPV were detected, among which, the detection rate of high-risk HPV in precancerous lesion group was77.85%, which was significantly lower than that in cervical cancer group at 94.12%, and the difference was of statistical significance(P<0.01). The relative expression of HPV and viral load in patients with precancerous lesions were significantly lower than those in cervical cancer patients(P<0.01). The high-risk subtypes of HPV were mainly HPV16 and HPV18, accounting for 34.75% and 20.73%, respectively. The positive rates of different subtypes in precancerous lesions group were significantly lower than those in cervical cancer group(P<0.01). Conclusion High-risk HPV infection is closely related to cervical precancerous lesions and cervical cancer, and is the main cause of cervical precancerous lesions and cervical cancer, therefore, early detection of HPV plays an important role in the prevention and treatment of cervical precancerous lesions and cervical cancer.
