血清CA125、NSE及24h尿VMA检测在小儿神经母细胞瘤诊治中的应用价值

【摘要】 目的探究血清糖类抗原125(CA125)、神经元特异性烯醇化酶(NSE)及24 h尿3-甲氧基-4羟基扁桃酸(VMA)在小儿神经母细胞瘤诊治中的应用价值。方法选取58例神经母细胞瘤患儿,采用电化学发光免疫分析法测定了患儿的血清CA125、NSE水平,采用高效液相色谱仪测定了患儿的24 h尿VMA水平。对不同临床分期、近期疗效神经母细胞瘤患儿的血清CA125、NSE及24 h尿VMA水平进行比较。结果 58例神经母细胞瘤患儿的血清CA125、NSE及24 h尿VMA阳性率分别为96.6%、81.0%、89.7%;临床分期为Ⅳ期的神经母细胞瘤患儿的血清CA125、NSE及24 h尿VMA水平均明显高于临床分期为Ⅱ~Ⅲ期的患儿(P﹤0.01);治疗有效组患儿的血清CA125、NSE及24 h尿VMA水平均明显低于治疗无效组(P﹤0.01)。结论血清CA125、NSE及24 h尿VMA水平可以在一定程度上评估小儿神经母细胞瘤患儿的病情和治疗效果。

【Abstract】 Objective To investigate the application value of the serum carbohydrate antigen 125(CA125), neuronspecific enolase(NSE) and the vanillylmandelic acid(VMA) in 24-hour urine in diagnosis and treatment of pediatric neuroblastoma. Method A total of 58 children with neuroblastoma were collected. The serum level of CA125 and NSE were detected using electrochemiluminescence assay, and 24-hour VMA level by high performance liquid chromatography(HPLC). The serum CA125, NSE and 24-hour urine VMA level in children patients with different clinical stage and varying short-term clinical efficacy were compared. Result The positive rate of serum CA125, NSE and 24-hour urine VMA in 58 children were 96.6%, 81.0% and 89.7%, respectively; patients with clinical stage IV disease had significantly higher level of serum CA125, NSE and 24-hour urine VMA compared with those with stage II-III disease(P<0.01); besides, patients who responded to therapy were with lower level of serum CA125, NSE and 24-hour urine VMA than those who did not(P<0.01). Conclusion Serum CA125, NSE and 24-hour urine VMA level are applicable to assess the condition and efficacy of children with neuroblastoma to some extent.
