放射性碘粒子植入对原发性肝癌患者免疫功能及治疗效果的影响
Effect of radioactive iodine particle implantation on the immune function of patients with primary liver cancer and the therapeutic effect
【摘要】 目的探讨放射性碘粒子植入对原发性肝癌患者免疫功能及治疗效果的影响。方法选取80例原发性肝癌患者作为研究对象。依据随机数字表法将患者随机分为研究组(n=40)和对照组(n=40),对照组患者接受肝癌切除术,研究组患者在肝癌切除术的基础上联合放射性碘粒子植入治疗。采用酶联免疫吸附试验(ELISA)检测并比较两组患者治疗前后的血清肿瘤标志物[甲胎蛋白(AFP)]水平,比较两组患者治疗后的肿瘤复发情况、免疫功能及肝功能。结果治疗后2、4、6周,两组患者的血清AFP水平均低于本组治疗前(P﹤0.05),且研究组患者的血清AFP水平明显低于对照组(P﹤0.01)。治疗后6、12、18个月,研究组患者的肿瘤复发率分别为2.5%、17.5%、45.0%,对照组患者的肿瘤复发率分别为22.5%、40.0%、85.0%。治疗后,研究组患者的CD4+/CD8+水平高于本组治疗前,对照组患者的CD4+/CD8+水平低于本组治疗前(P﹤0.05)。治疗后,研究组患者的血清CD4+及CD4+/CD8+水平均高于对照组患者,CD8+水平低于对照组患者(P﹤0.05)。治疗后,两组患者的肝功能指标水平均高于本组治疗前(P﹤0.05)。结论在肝癌切除术的基础上联合放射性碘粒子植入治疗对于原发性肝癌患者具有较好的治疗效果,可更有效地杀灭肝癌细胞,预防肝癌复发,并改善免疫功能,具有良好的应用价值。
【Abstract】 Objective To study the effect of radioactive iodine particle implantation on the immune function of patients with primary liver cancer and to evaluate the therapeutic effect. Method 80 cases with primary liver cancer were enrolled and randomized into study group(n=40) and control group(n=40) using a random number table, hepatectomy was administered for control group, while the study group was additionally given radioactive iodine particle implantation based on hepatectomy. Enzyme-linked immunosorbent assay(ELISA) was utilized to compare the level of serum tumor marker(alpha-fetal protein, AFP) in the two groups before and after treatment, besides, the tumor recurrence, immune function and liver function were also compared. Result In 2, 4 and 6 weeks after treatment, the serum AFP in both groups were lower than those before treatment(P<0.05), and a significantly lower level of serum AFP was observed in study group than in control group(P<0.01). In 6, 12 and 18 months, the recurrence rates in study group were 2.5%,17.5%, 45.0%, the recurrence rates in control group were 22.5%, 40.0%, 85.0%. After treatment, the CD4+/CD8+level in study group was increased, while the CD4+/CD8+level in the control group was lower than that before treatment(P<0.05). The serum level of CD4+and CD4+/CD8+were significantly higher, and CD8+was lower in study group compared with control group(P<0.05). In addition, the liver function index in both groups were improved after treatment(P<0.05).Conclusion The combined therapy of adding radioactive iodine particle implantation to hepatectomy is clinically effective for the treatment of primary liver cancer, with high efficiency of killing liver cancer cells, preventing tumor recurrence and ameliorating immune function, showing clinical significance.
