【摘要】 目的:探讨掌侧锁定钢板与外固定架治疗复杂性桡骨远端骨折的疗效。方法:43例单侧不稳定性桡骨远端骨折患者,其中掌侧锁定钢板组(A组)25例,外固定架组(B组)18例。通过骨折愈合时间、视觉模拟评分(VAS)、肩肘手残疾评分(DASH)、影像学指标和腕关节活动度、手握力比较两组患者术后疗效。结果:随访16～23个月,X线片提示骨折均愈合,两组骨折愈合时间、VAS疼痛评分、腕关节活动度及手握力差异无统计学意义(P>0.05);A组DASH评分为(8.4±4.4)分,B组为(16.6±9.1)分;影像学指标,A组掌倾角和桡骨高度的恢复更佳(P<0.05)。而两组桡偏角差异无统计学意义(P>0.05)。A组患者未出现明显术后并发症,B组有3例患者术后出现钉道感染。结论:掌侧锁定钢板组在DASH评分与外固定架组相比有明显差异,且其在掌倾角和桡骨高度恢复有优势。更多还原

【Abstract】 Objective:To compare the outcomes of open reduction and internal fixation with volar locking plate and external fixation in treating complex distal radius fracture.Methods:43 patients with unstable distal radius fracture were divived into volar locking plate group(A group n=25)and external fixation group(B group n=18).The two groups were compared for clinical and functional outcomes measured by fracture healing time,visual analogue scale(VAS),the disabilities of the arm,shoulder,and hand(DASH)score, radiographic measurements and grip strength were also evaluated between groups.Result:43 cases were all follow-up,follow-up period was 16-23 months.All fractures were achieved,final ranges of motion and grip strengths were similar betweeen the two groups(P>0.05).The DASH score of A group was(8.4±4.4) to(16.6±9.1) of B group.Radiographically,valar tilt angle and radial height were significantly better in the A group(P<0.05), radial inclination angle was no significant difference between two groups. No complications occurred in the A group whereas 3 patients had pin tract infections in the B group. Conclusion:locked volar plating gave better DASH score and some radiographic parameters than external fixation. Final ranges of motion and grip strengths were similar between the two groups.更多还原
