【摘要】 目的:观察三维外支架微创治疗老年股骨转子间骨折的疗效。方法:对2015年3月-2017年2月福建中医药大学附属泉州市正骨医院下肢一科符合纳入标准的老年股骨转子间骨折患者110例进行随访分析,其中孟氏架组35例、三维外支架组37例及PFNA组38例,分别采用孟氏架、三维外支架或PFNA进行固定,对三组患者手术资料、辅助复位率、术后并发症、患肢下地部分负重时间、骨折愈合时间和髋关节功能评分等临床资料进行分析。结果:三维外支架组及孟氏架组手术时间、术中出血量较PFNA组明显减少,对比具有显著性差异(P <0.01),但三维外支架组及孟氏架组组间对比无明显差异(P> 0.05);三组患肢下地部分负重时间、骨折愈合时间比较,差异无统计学意义(P> 0.05);三维外支架组及PFNA组的优良率比孟氏架组明显提高,差异有统计学意义(P <0.05),而三维外支架组与PFNA组的优良率相当,差异无统计学意义(P> 0.05);三维外支架组及PFNA组的并发症比孟氏架组均明显减少,差异有统计学意义(P <0.05),但三维外支架组的并发症与PFNA组比较,差异无统计学意义(P> 0.05)。结论:三维外支架微创治疗老年股骨转子间骨折不仅微创、安全、经济,而且固定更牢靠,值得推广应用。更多还原

【Abstract】 Objective: To observe clinical efficacy of three-dimensional external stent minimally invasive on elderly intertrochanteric fracture. Methods: From March 2015 to February 2017, 110 cases were divided into three groups. 35 cases in the Meng’s support group were given Meng’s support treatment; 37 cases in the three-dimensional external stent group were given three-dimensional external stent treatment; and 38 cases in the PFNA group were given PFNA treatment. Results: The operation time and intraoperative blood loss in the Meng’s support group and the three-dimensional external stent group were significantly lower than those in the PFNA group(P<0.01); but there was no significant difference between the three-dimensional external stent group and the Meng’s support group(P>0.05). There was no significant difference in weight-bearing time and fracture healing time between three groups(P>0.05). The excellent and good rate in the three-dimensional external stent group and the PFNA group was significantly higher than the Meng’s support group(P<0.05). There was no significant difference in the excellent rate between the three-dimensional external stent group and the PFNA group(P>0.05). The complications in the three-dimensional external stent group and the PFNA group were less than the Meng’s support group(P<0.05). There was no significant difference in the complications between the three-dimensional external stent group and the PFNA group(P>0.05). Conclusion: Three-dimensional external stent minimally invasive on elderly intertrochanteric fracture show a good effect, and is worthy of promotion.更多还原
