氯胺酮依赖者戒断期血清NT-proBNP升高的原因探讨

【摘要】 目的:探究氯胺酮依赖者在强制戒断期血清NT-proBNP升高的原因。方法:采用Mini-VidasBlue全自动荧光免疫分析仪及其配套原装试剂进行血清N末端脑钠尿肽前体(NT-proBNP),以小于450pg/ml排除截点,对432例氯胺酮依赖患者进行检测。同时对可能影响血清NT-proBNP变化的吸毒时间、性别、年龄、肥胖、肾功能不全及心率加快、血压升高、喉痉挛、支气管痉挛呼吸困难、哮喘、肌强直、极度烦躁等因素进行分析。结果:戒断症状期与戒断症状消失期的血清NT-proBNP≥450ng/L检测值两者比较差异有统计学意义(t′=11.23,P<0.001)、血清NT-proBNP<450ng/L检测值两者比较差异有统计学意义(t′=5.273,P<0.001);吸食时间不同造成血清NT-proBNP检测值总体比较存在显著正相关(χ~2=7.13,P<0.001);临床症状不同的患者血压升高出现的频率存在显著正相关(χ~2=4.319,p<0.05)、喉痉挛出现的频率存在显著正相关(χ~2=22.218,p<0.001)、支气管痉挛呼吸困难出现的频率存在显著正相关(χ~2=21.984,p<0.001)、哮喘出现的频率存在显著正相关(χ~2=38.275,p<0.001)、肌强直出现的频率存在显著正相关(χ~2=18.862,p<0.001)、极度烦躁出现的频率无显著相关性。性别、年龄对血清中的NT-proBNP无影响(P>0.05);肥胖对血清中的NT-proBNP影响显著,差异有统计学意义(χ~2=22.554,P<0.001);肾功能异常对血清中的NT-proBNP影响显著,差异有统计学意义(χ~2=127.8,P<0.001)。结论:氯胺酮依赖者戒断症状期由于体内毒品代谢异常是导致血浆NT-proBNP显著升高的主要原因,肾功能不全、吸毒时间的差异及血压升高、喉痉挛、支气管痉挛呼吸困难、哮喘、肌强直可能是导致血清NT-proBNP升高的关联因素。

【Abstract】 Objective: To investigate the causes of elevated NT-proBNP in ketamine-dependent patients during the forced withdrawal period. Methods: Serum-N-terminal pro-brain natriuretic peptide precursor(NT-proBNP) was performed using a Mini-Vidas-Blue automatic fluorescence immunoassay analyzer and its associated original reagents. Exclusion points were excluded at less than 450 pg/ml, and 432 patients with ketamine dependence were examined.. At the same time, factors such as gender, age, obesity, renal insufficiency and increased heart rate, high blood pressure, throat, bronchospasm dyspnea, asthma, muscle rigidity, and extreme irritability affect serum NT-proBNP changes. Results: There were significant differences in serum NT-proBNP/450 ng/L between the withdrawal symptoms stage and the withdrawal stage of withdrawal symptoms(t′=11.23, P<0.001), serum NT-proBNP<450 ng/L. There was a statistically significant difference between the two groups(t ′=5.273, P <0.001); there was a significant positive correlation between the total serum NT-proBNP values(χ 2=7.13, P <0.001), and patients with different clinical symptoms. There was a significant positive correlation between the frequency of blood pressure rises(χ2=4.319, P<0.05), a significant positive correlation between the frequency of throat spasm(χ2=22.218, P <0.001), and a significant positive correlation between the frequency of bronchospasm dyspnea.(χ 2 = 21.984, P < 0.001), there was a significant positive correlation between the frequency of asthma(χ 2 = 38.275, P <0.001), there was a significant positive correlation of the frequency of myotonia(χ2 = 18.862, P <0.001), extremely irritated There was no significant correlation with frequency. Gender and age had no effect on NT-proBNP in serum(P >0.05); Obesity had significant effect on NT-proBNP in serum, and the difference was statistically significant(χ 2=22.554, P <0.001); renal function abnormalities were found in serum. The effect of NT-proBNP was significant and the difference was statistically significant(χ2=127.8, P<0.001). Conclusion: Ketamine dependence in the withdrawal phase due to abnormal drug metabolism in the body is the main cause of significant increase in plasma NT-proBNP, renal insufficiency, the difference in drug time and blood pressure, throat, bronchial spasms, difficulty in breathing, asthma, muscle rigidity Different symptoms are the cause of elevated serum NT-proBNP.
