北京市首例长湾泥沙门菌的鉴定及溯源

【摘要】 目的研究北京市首次发现的长湾泥沙门菌株的识别及分子生物学特征,提供新的病原学依据,为沙门菌新的血清型别的鉴定、溯源及流行病学研究提供参考。方法查阅患儿就诊记录和回访进行流行病学调查,采集相关样本进行培养检测,采用系统生化鉴定、血清分型鉴定,聚合酶链式反应(PCR)技术复核鉴定,脉冲场凝胶电泳(PFGE)分型分析,革兰阴性需氧菌药敏检测板进行药敏试验。结果临床分离菌株47个生化试验中23个生化试验为阴性,24个生化反应为阳性,符合典型沙门菌反应,菌株血清鉴定及PCR测序后序列比对均符合长湾泥沙门菌株的表型特征(Salmonella Jangwani,17:a:1,5),PFGE分型与浙江省余姚市检出的长湾泥沙门菌带型相似度100%;相关样本中检出新加坡沙门菌,此两种血清型沙门菌PFGE分型分属于两个克隆,两种血清型沙门菌对检测的15种抗菌药物均敏感。结论菌株为北京市首次检出的血清型沙门菌,为本地区传染病防治提供了新的病原学依据,但是此次溯源分析尚不能直接证明患儿此次感染与宠物龟接触有关。

【Abstract】 [Objective] To provide a reference for identification, traceability and epidemiological research of Salmonella new serotype by studying the identification and molecular biological characteristics of the first strain of Salmonella changwanni isolation in Beijing. [Methods] Systematic biochemical identification, serotype identification and polymerase chain reaction(PCR) were conducted to identify the strain, the molecular subtyping was analyzed by Pulsed Field Gel Electrophoresis(PFGE).The drug sensitive bacteria Gram-negative aerobic plate detection was used for the detection of drug resistance, and epidemiological survey through sick child medical record review and follow up was conducted. [Results] There were 23 negative biochemical reactions and 24 positive reactions of the 47 biochemical tests which consistent with the typical reaction of Salmonella strains. Serological identification and PCR sequencing showed that the isolated strain had the phenotypic characteristics(Salmonella Jangwani, 17 ∶a ∶1,5). PFGE typing revealed that the belt type has 100% similarity to Salmonella changwanni isolated in Yuyao City of Zhejiang Province. The Salmonella Singapore was isolated from the related samples. Two serotypes of Salmonella belong to two clones, and they were sensitive to 15 antibiotics. [Conclusion] It is the first time to detect this serotype of Salmonella in Beijing. It provides a new pathogeny basis for the prevention and control of infectious diseases in the region. But it can not prove the direct relationship with the contact of the pet turtles.
