【摘要】 目的探讨门诊随访结合QQ群平台在慢性肾脏病(CKD)延续护理中的应用效果。方法选择2016年5月—2017年5月纳入CKD管理中心的患者100例为研究对象。专职CKD管理护士负责档案管理、数据库管理、患者随访、患者教育工作,同时建立QQ群,由本人或家属加入QQ群,利用QQ群追踪随访管理和患者教育工作。结果入组时CKD患者的疾病认知问卷评分为(29.20±9.78)分,入组3个月后为(70.00±8.71)分,差异有统计学意义(P<0.01)。结论对CKD患者实施门诊随访结合QQ群平台的延续护理有良好的效果,能够提高CKD患者对疾病的认知。更多还原

【Abstract】 Objective To evaluate the outpatient follow-up combined with QQ group chatting in continuing nursing care of chronic kidney disease( CKD). Methods Totally 100 patients registered in the CKD Management Center from May,2016 to May,2017 were selected as the study subjects. Full-time CKD nurses were responsible for file management,database management,patient follow-up,patient education,and establishment a QQ online chatting group. The QQ group was composed of patients and their families. The follow-up management and patient education were tracked through the QQ group. Results Three months after intervention,the patients achieved a higher outcome in disease awareness assessment compared with that before intervention(70. 00 ± 8. 71 vs. 29. 20 ± 9. 78,P < 0. 01). Conclusion Implementation of outpatient followup combined with QQ group chatting plays an important role in continuing nursing of CKD.
