9910例患者和献血员Rh表型检测与临床意义
Rh phenotype detection and its clinical significance in 9910 patients and blood donors
【摘要】 目的统计患者和献血员Rh表型的分布规律,分析Rh表型匹配输注在临床输血中的意义。方法采用Rh血型抗原检测卡检测患者Rh表型,采用Galileo全自动血型分析仪检测患者不规则抗体,并进一步确定抗体类型。结果4 392例患者样本中,检出D抗原阴性样本16例,阴性率为0.36%。患者和献血员Rh系统5个主要抗原频率基本一致:D>e>C>c>E。患者和献血员Rh表型分布规律基本一致:CCDee>Cc DEe>Cc Dee>cc DEE>cc DEe>CCDEe>cc Dee>Cc DEE,均未发现CCDEE表型。Rh表型配合输注率为89.08%(4 471/5 019),随访均未产生Rh系统抗体。输血前4 392例患者筛查出不规则抗体28例,其中特异性抗体23例,主要为Rh系统,占73.91%(17/23)。结论建立患者和献血员的Rh表型库,选用表型匹配的红细胞输注,可减少Rh抗体的产生,降低输血不良反应的发生率,减少交叉配血时间,保障输血安全。
【Abstract】 Objective To investigate the distribution pattern of Rh phenotype in patients and blood donors,and to evaluate the significance of Rh phenotype infusion in clinical blood transfusion. Methods The Rh phenotype was detected by Rh blood group antigen test card. Galileo automatic blood group analyzer was used to detect irregular antibodies,and the type of antibody was further detected. Results In the samples from 4 392 patients,Rh( D) negative was detected in 16 cases,and the rate was0. 36%. The 5 main antigen frequencies of the Rh system antigen were almost consistent between the patients and the donors:D > e > C > c > E. The distribution pattern of Rh phenotypes in patients and donors was basically the same: CCDee > Cc DEe >Cc Dee > cc DEE > cc DEe > CCDEe > cc Dee > Cc DEE,and no CCDEE phenotype was detected. The rate of Rh phenotype matching was 89. 08%( 4 471/5 019),and no Rh system antibodies were produced in follow-up. 28 cases of irregular antibodies were detected in 4 392 cases,of which 23 cases were specific antibodies,mainly in Rh system,accounting for 73. 91%( 17/23). Conclusion In the establishment of Rh patients and blood donors phenotype database,using the same phenotype blood transfusion may reduce the production of Rh antibodies and the incidence rate of adverse reactions in patients with blood transfusion,reduce the blood cross matching time and promote the safety of blood transfusion.
