CRP、PCT在新生儿细菌感染性黄疸早期诊断中的应用价值

【摘要】 目的探讨血清C反应蛋白(CRP)、降钙素原(PCT)在新生儿细菌感染性黄疸早期诊断中的应用价值。方法选取2013年1月-2016年10月医院生产的细菌感染性黄疸患儿75例作为感染性黄疸组,选取同期生产的新生儿生理性黄疸患儿62例作为生理性黄疸组,检测感染性黄疸组治疗前后及生理性黄疸组CRP、PCT水平及经皮胆红素值(TcB),并分析各指标间的关系。结果感染性黄疸组治疗前CRP、PCT、TcB水平显著高于生理性黄疸组(P<0.05);感染性黄疸组中,轻度组治疗前CRP、PCT、TcB低于中度组、重度组,中度组低于重度组(P<0.05);轻度组治疗10dPCT水平低于中度组、重度组,中度组低于重度组,差异有统计学意义(P<0.05),3组治疗10dCRP比较差异无统计学意义(P>0.05)。Pearson相关分析显示,TcB与CRP、PCT呈正相关关系(r=0.412、0.543,P<0.05)。结论 CRP、PCT可作为辅助诊断新生儿细菌感染性黄疸的重要指标。

【Abstract】 Objective To investigate the application value of serum C reactive protein(CRP)and proealcitonin(PCT)in early diagnosis of bacterial infectious jaundice in neonates.Methods 75 neonates with bacterial infectious jaundice born in the hospital during January 2013-October 2016 were selected as the infectious jaundice group,and another 62 neonates with physiological jaundice were selected as the physiological jaundice group.The levels of CRP and PCT and transcutaneous bilirubin(TcB)in the two groups were detected before and after the treatment.The relationship between indexes was analyzed.Results Levels of CRP,PCT and TcB in the infectious jaundice group were significantly higher than those in the physiological jaundice group(P<0.05).In the infectious jaundice group,levels of CRP,PCT and TcB showed mild group<moderate group<severe group before treatment(P<0.05).After 10 dof treatment,the level of PCT showed mild group<moderate group<severe group(P<0.05),but there was no significant difference in CRP among the 3 groups after 10 dof treatment(P>0.05).Pearson correlation analysis showed that TcB was positively correlated with CRP and PCT(r=0.412,0.543,P<0.05).Conclusion CRP and PCT can be used as important indicator for the diagnosis of neonatal bacterial infectious jaundice.
