【摘要】 目的探讨帝视内窥镜引导气管插管在甲状腺手术中的应用价值。方法 100例行双侧甲状腺次全切除手术患者,随机分为研究组(58例)与对照组(42例)。研究组患者采用帝视内窥镜引导气管插管,对照组患者采用Macintosh直接喉镜引导气管插管。比较两组患者一次插管成功率、平均插管时间及麻醉诱导前(T0)、麻醉诱导后(T1)、插管时(T2)的平均动脉压(MAP)、心率(HR)。结果研究组插管一次性成功率为96.55%,明显高于对照组的80.95%,差异具有统计学意义(χ2=6.586,P<0.05)。研究组患者平均插管时间短于对照组,差异具有统计学意义(t=5.766,P<0.05)。两组患者T0和T1 MAP和HR比较差异均无统计学意义(P>0.05);研究组患者T2时MAP与HR均低于对照组,差异具有统计学意义(P<0.05)。结论采用帝视内窥镜引导气管插管一次插管成功率比较高,平均插管时间短,循环波动小,有助于甲状腺手术患者气道位置的判断,值得在临床上使用。更多还原

【Abstract】 Objective To explore the application value of tracheal intubation guided by disposcope endoscope in thyroid surgery. Methods A total of 100 patients with bilateral subtotal thyroidectomy were randomly divided into research group for 58 cases and control group for 42 cases. Patients in the research group were given tracheal intubation guided by disposcope endoscope, patients in the control group were given tracheal intubation guided by Macintosh direct laryngoscope. The one time intubation success rate, mean intubation time, mean arterial pressure(MAP) and heart rate(HR) before anesthesia induction(T0), after anesthesia induction(T1) and at intubatton(T2) between the two groups were compared. Results The one time success rate of intubation in the research group was 96.55%, which was significantly higher than 80.95% in the control group, the difference was statistically significant(χ2=6.586, P<0.05). The mean intubation time of the research group was significantly lower than that of the control group, the difference was statistically significant(t=5.766, P<0.05). There were no significant differences in the MAP and HR at T0 and T1 between the two groups(P>0.05), the MAP and HR of the research group at T2 were significantly lower than those of the control group, the differences were statistically significant(P<0.05). Conclusion In the application of tracheal intubation guided by disposcope endoscope, the one time success rate of intubation is high, the mean intubation time is short and the circulation fluctuation is small. It is helpful to judge the position of the airway in the patients with thyroid surgery, which is worthy of clinical use.更多还原
