【摘要】 目的探讨腹腔镜阑尾切除术治疗特殊类型阑尾炎的临床效果。方法 82例特殊类型阑尾炎患者,按照随机数字表法分为对照组和观察组,每组41例。对照组患者行传统开腹手术治疗,观察组患者行腹腔镜阑尾切除术治疗。比较两组手术时间、手术出血量、术后下床活动时间、住院时间、术后并发症发生情况及疼痛情况。结果观察组手术时间短于对照组,手术出血量少于对照组,视觉模拟评分法(VAS)评分低于对照组,差异具有统计学意义(P<0.05);两组术后下床活动时间、住院时间比较差异无统计学意义(P>0.05)。观察组术后并发症发生率为7.32%,低于对照组的39.02%,差异具有统计学意义(P<0.05)。结论对特殊类型阑尾炎患者采取腹腔镜阑尾切除术治疗的效果较好,具有创伤小、疼痛轻、并发症少等优点,值得应用。更多还原

【Abstract】 Objective To investigate the clinical effect of laparoscopic appendectomy for the treatment of special type appendicitis. Methods A total of 82 special type appendicitis patients were divided by random number table method into control group and observation group, with 41 cases in each group. The control group received traditional laparotomy for treatment, and the observation group received laparoscopic appendectomy for treatment. Comparison were made on operation time, surgical bleeding volume, postoperative off-bed activity time, hospitalization time, occurrence of postoperative complications and pain condition between the two groups. Results The observation group had shorter operation time than the control group, less surgical bleeding volume than the control group, and lower visual analogue scale(VAS)score than the control group. Their difference was statistically significant(P<0.05). Both groups had no statistically significant difference in postoperative offbed activity time and hospitalization time(P>0.05). The observation group had lower incidence of postoperative complications as 7.32% than 39.02% in the control group, and the difference was statistically significant(P<0.05). Conclusion Application of laparoscopic appendectomy shows better effect for patients with special type appendicitis, and it has the advantages of small trauma, light pain and less complications.
