急性期脑卒中患者血糖水平及临床结局的相关性研究
Correlation between Blood Glucose Level and CLinical Outcome in Patients with Acute Stroke
【摘要】 目的探讨急性期脑卒中患者血糖水平和临床结局的相关性。方法选取我院2015年5月至2017年5月神经内科收治的120例患者作为研究对象,所有患者均经电子计算机断层成像(CT)或核磁共振成像(MRI)确诊为急性脑卒中,入院24h内检测患者血糖水平,于患者出院或死亡时评估其临床结局,分析血糖水平与临床结局的相关性。结果 120例患者中,痊愈患者共计59例,残疾患者共计37例,死亡患者共计24例。120例患者中,痊愈、残疾和死亡三种临床结局的患者之间血糖水平不同。死亡组病例的血糖水平高于残疾组和痊愈组,残疾组病例的血糖水平高于痊愈组。随着入院血糖水平的升高,急性期脑卒中患者发生死亡的风险有升高的趋势(P<0.05)。急性期脑卒中患者发生残疾的风险无明显变化(P>0.05)。结论于脑卒中患者发病早期检测血糖对判断患者病情、指导治疗措施和改善患者预后具有重要参考作用。
【Abstract】 Objective To investigate the correlation between blood glucose level and clinical outcome in acute stroke patients. Methods A total of 120 patients admitted to Department of Neurology from May 2015 to May 2017 in our hospital were enrolled in this study. All patients were diagnosed as acute stroke by computed tomography(CT) or magnetic resonance imaging(MRI) The patient’s blood glucose level was measured during the 24 hour, the clinical outcome was assessed at the time of discharge or death, and the correlation between blood glucose level and clinical outcome was analyzed. Results Of 120 patients, 59 were cured, 37 were disabled and 24 died. Blood glucose levels differ among patients with three clinical outcomes: recovery, disability and death. The blood glucose level in the death group was higher than that in the disability group and the cured group, and the blood sugar level in the disability group was higher than that in the cured group. With the rise of blood glucose levels on admission, the risk of death in patients with acute stroke has an increasing trend(P<0.05). No significant changes in the risk of disability in patients with acute stroke(P>0.05). Conclusion Early detection of blood glucose in stroke patients plays an important role in judging the patient’s condition, guiding the treatment and improving the prognosis of the patient.
