彩色多普勒高频超声联合血清肿瘤标志物对乳腺癌的诊断价值

【摘要】 目的探讨彩色多普勒高频超声联合血清肿瘤标志物糖原153(CA153)和糖原199(CA199)对乳腺癌的诊断价值。方法选取2014年1月至2016年1月间湖北省鄂州市中心医院收治的116例乳腺癌患者作为乳腺癌组,同期收治的284例良性乳腺疾病患者作为良性乳腺疾病组,同期湖北省鄂州市中心医院体检中心体检的200例健康女性作为对照组,应用电化学发光分析法检测3组受试者血清CA153和CA199水平,对3组受试者进行乳腺彩色多普勒高频超声检查,比较彩色多普勒高频超声联合血清CA153+CA199与单独彩色多普勒高频超声检查、单独血清CA153+CA199检查对乳腺癌诊断的敏感度、特异性。结果乳腺癌组CA199及CA153水平均显著高于对照组和良性乳腺疾病组,良性乳腺疾病组CA199及CA153水平显著高于对照组,差异均有统计学意义(均P<0.05)。单独血清CA153+CA199检测乳腺癌的敏感度为80.2%,特异性为87.4%;单独彩色多普勒高频超声检测乳腺癌的敏感度为86.2%,特异性为89.1%。血清CA153+CA199联合彩色多普勒高频超声检测乳腺癌的敏感度为95.7%,特异性为96.1%,均显著高于单独血清CA153+CA199和单独彩色多普勒高频超声检测乳腺癌,差异均有统计学意义(均P<0.05)。结论彩色多普勒高频超声联合血清CA153、CA199检测乳腺癌可有效提高敏感度及特异性,有助于乳腺癌的诊断、监测及治疗。

【Abstract】 Objective To investigate the value of high-frequency color Doppler ultrasound combined with serum carbohydrate antigen 153( CA153) and carbohydrate antigen 199( CA199) in the diagnosis of breast cancer. Methods One hundred and sixteen patients with breast cancer treated at Ezhou Central Hospital from January 2014 to January 2016 were selected as the breast cancer group,284 patients with benign breast disease were selected as benign breast disease group and in the same period 200 healthy women who had physical examination were selected as the control group. Electrochemiluminescence analysis was used for the detection of serum CA153 and CA199 levels in the three groups. The sensitivity and specificity of high-frequency color Doppler ultrasound combined with serum CA153 and CA199 detection and single high-frequency color Doppler ultrasound and serum CA153 and CA199 in the diagnosis of breast cancer were compared. Results The CA153 and CA199 levels were significantly higher in the breast cancer group than in the benign breast disease group and the control group and the CA153 and CA199 levels were significantly higher in the benign breast disease group than in the control group( all P < 0. 05). The sensitivity of serum CA153 and CA199 in the detection of breast cancer was 80. 2% and the specificity was 87. 4%. The sensitivity of high-frequency color Doppler ultrasound detection in the detection of breast cancer was 86. 2%and the specificity was 89. 1%. The sensitivity of high-frequency color Doppler ultrasound combined with serum CA153 and CA199 in the detection of breast cancer was 95. 7% and the specificity was 96. 1%( all P < 0. 05). Conclusion Serum CA153 and CA199 combined with high-frequency color Doppler ultrasound in the detection of breast cancer can improve the specificity and sensitivity,which can promote the diagnosis,treatment and dection of breast cancer.
