血C反应蛋白、纤维蛋白原、白介素-6水平与急性脑梗死牛津郡社区卒中计划分型的相关性研究
Study on correlation between serum C-reactive protein, fibrinogen, interleukin-6 level and Oxford County Community stroke plan classification in acute cerebral infarction
【摘要】 目的研究急性脑梗死患者牛津郡社区卒中计划(OCSP)分型与血清C反应蛋白(CRP)、纤维蛋白原(Fig)以及白介素-6(IL-6)水平的关系。方法选取我院2016年10月~2018年5月收治的120例急性脑梗死患者作为研究对象,所有患者根据OCSP分型分为完全前循环梗死(TACI)组、部分前循环梗死(PACI)组、后循环梗死(POCI)组以及腔隙性梗死(LACI)组,每组各30例。另选取同期来我院进行体检的30例健康志愿者作为对照组。检测4个亚型患者及对照组的CRP、Fig以及IL-6水平,观察不同亚型患者及对照组的神经功能情况,分析急性脑梗死OCSP分型、神经功能情况与CRP、Fig以及IL-6水平的关系。结果 TACI组患者的NIHSS评分最高,明显高于PACI、POCI、LACI组以及对照组,差异有统计学意义(P<0.05)。PACI组与POCI组患者的NIHSS评分比较,差异无统计学意义(P>0.05)。PACI组与POCI组患者的NIHSS评分均高于LACI组和对照组,LACI组患者的NIHSS评分高于对照组,差异有统计学意义(P<0.05)。TACI组患者的CRP、Fig以及IL-6水平明显高于PACI、POCI、LACI组以及对照组,差异有统计学意义(P<0.05)。PACI组与POCI组患者的CRP、Fig以及IL-6水平比较,差异无统计学意义(P>0.05)。PACI组与POCI组患者的CRP、Fig以及IL-6水平均高于LACI组和对照组,LACI组患者的CRP、Fig以及IL-6水平高于对照组,差异有统计学意义(P<0.05)。NIHSS评分越高,OCSP分型越严重,CRP、Fig以及IL-6水平越高,NIHSS评分、OCSP分型与CRP、Fig以及IL-6水平成明显正相关(均P<0.05)。结论随着CRP、Fig以及IL-6水平的升高,急性脑梗死患者病情程度越严重,预后越差。CRP、Fig以及IL-6可能参与了急性脑梗死的发展过程。
【Abstract】 Objective To study the relationship between the Oxfordshire Community stroke project(OCSP) classification and the serum C-reactive protein(CRP), fibrinogen(Fig) and interleukin-6(IL-6) level in acute cerebral infarction. Methods Altogether 120 patients with acute cerebral infarction treated in our hospital from October 2016 to May2018 were enrolled in this study and divided into the total anterior circulation infarction(TACI) group, the partial anterior circulation infarction(PACI) group, the posterior circulation infarction(POCI) group and the lacunar infarct(LACI)group according to the OCSP classification, with 30 cases in each group. Another 30 healthy volunteers who underwent medical examinations in our hospital during the same period were selected as the control group. The levels of CRP, Fig and IL-6 of four subtypes and the control group were detected, and the neurological function of different subtypes and the control group were observed. The relationship between OCSP classification, neurological function and CRP, Fig and IL-6 levels in acute cerebral infarction were analyzed. Results The NIHSS score of TACI group was highest, which was significantly higher than that of the PACI group, the POCI group, the LACI group and the control group, with significant difference(P<0.05). There was no significant difference in the NIHSS score between the PACI group and the POCI group(P>0.05), the NIHSS score in the PACI group and the POCI group were higher than those in the LACI group and the control group, the NIHSS score in the LACI group was higher than that in the control group, with significant difference(P<0.05). The CRP, Fig and IL-6 levels of the TACI group were higher than those of the PACI group,the POCI group, the LACI group and the control group, with significant difference(P<0.05). There was no statistically significant difference in the level of CRP, Fig and IL-6 between the PACI group and the POCI group(P>0.05), the level of CRP, Fig and IL-6 of the PACI group and the POCI group were higher than those of the LACI group and the control group, the level of CRP, Fig and IL-6 in the LACI group was higher than that in the control group, with significant difference(P<0.05). The higher the NIHSS score was, the more serious the OCSP classification was, and the higher the CRP, Fig and IL-6 levels were. The NIHSS score, the OCSP classification and the CRP, Fig and IL-6 were positively correlated(all P<0.05). Conclusion With the increase of CRP, Fig and IL-6 levels, the more serious the severity of acute cerebral infarction is, the worse the prognosis is. CRP, Fig and IL-6 may be involved in the development of acute cerebral infarction.
