血清可溶性低密度脂蛋白受体11水平与2型糖尿病及颈动脉内-中膜厚度关系的研究

【摘要】 目的探讨血清可溶性低密度脂蛋白受体11(sLR11)水平与T2DM及颈动脉内-中膜厚度(CIMT)的关系及其临床意义。方法选取新诊断T2DM患者(T2DM组)51例及同期健康体检者(NC组)55名,ELISA测定血清sLR11水平,比较两组sLR11、CIMT、年龄、血脂及血压。另选常规治疗的T2DM患者165例,分析sLR11、CIMT与动脉硬化危险因素之间的关系,以及CIMT进展中sLR11与FPG的影响。结果 T2DM组血清sLR11、CIMT均高于NC组[(8.35±2.65)vs(6.64±1.99)ng/ml,P=0.026;(0.87±0.37)vs(0.73±0.26)mm,P=0.037]。165例T2DM患者中,血清sLR11水平为(9.79±3.54)ng/ml,CIMT为(0.79±0.18)mm。相关分析显示,sLR11水平与年龄、SBP、LDL-C、FPG、HbA1c呈正相关(r=0.226、0.162、0.166、0.276、0.176,P<0.05);多元线性回归分析显示,FPG与血清sLR11独立相关(β=0.175,P=0.023);不考虑年龄因素时,sLR11是CIMT的独立危险因素(β=0.008,P=0.044)。结论血清sLR11可能与T2DM血糖相关,并反映CIMT情况及血管内膜平滑肌细胞病理变化。

【Abstract】 Objective To investigate the relationship and clinical significance between circulating soluble low-density lipoprotein receptor family member 11(sLR11) level, type 2 diabetes(T2 DM) and carotid intima-media thickness(CIMT). Methods Newly diagnosed patients with T2 DM(T2 DM group, n=55) and healthy controls(NC group, n=55) were enrolled in this study. The sLRll level was measured by ELISA. The level of sLRll, CIMT, age, blood lipid, and blood pressure were compared between the two groups. Another 165 patients with T2 DM treated with conventional pharmaceutical medications were also.selected in this study. The relationship between sLRll, CIMT and atherosclerosis were analyzed. The influences of sLRll and FPG in the progress of CIMT were also evaluated. Results The sLR11 level and CIMT were higher in T2 DM group than in NC group [(8. 35±2. 65)vs(6. 64±1.99) ng/ml,P = 0. 026;(0.87±0.37) vs(0.73±0.26) mm,P=0.037]. In the 165 patients with T2 DM,the level of sLR11 and CIMT were(9. 79 ± 3. 54) ng/ml and(0. 79± 0.18) mm respectively.Correlation analysis showed that sLRll levels were positively correlated with age, systolic blood pressure,LDL-C, fasting plasma-glucose(FPG), and HbA1 c(r= 0.226, 0.162, 0.166,0.276, 0.176).Multivariate linear regression analysis indicated that FPG was an independent risk factor for sLRll(β=0.175, P<0. 05), and sLRll was a independent risk factor of CIMT in the absence of age factor(β=0. 008, P<0. 05). Conclusion The sLRll level may relate to blood glucose in T2 DM, and reflect the situation of CIMT and the pathological status of vessel intimal smooth muscle cells(SMCs).
