高频术中探头联合超声造影在早期类风湿关节炎掌指关节滑膜病变的应用
The Value of Ultra-high Frequency Intraoperative Probe Combined with Ultrasonography in the Diagnosis of Metacarpophalangeal Synovium of Rheumatoid Arthritis
【摘要】 目的采用高频术中探头联合超声造影评估类风湿关节炎(RA)患者掌指关节(MCP)滑膜病变。方法对154例未经治疗的早期RA患者1 540个MCP关节行超声检查,选取滑膜增生的MCP关节行超声造影检查,比较二维图像及超声造影时关节滑膜的厚度,分析超声造影时滑膜厚度变化与能量多普勒血流分级的相关性。结果 431个(431/1 540,28%)MCP出现不同程度的滑膜增生,其中背侧滑膜增生319个(74%),掌侧滑膜增生77个(17.9%),双侧滑膜增生35个(8.1%);二维图像测量滑膜厚度为2.3±1.6,超声造影时测量滑膜厚度为2.7±2.1。结论高频术中超声探头联合超声造影对早期RA患者掌指关节滑膜增生病变判断的灵敏度明显提高,减少了测量误差。
【Abstract】 Objective Ultra-high Frequency Intraoperative Probe combined with contrast echocardiography was used to assess metacarpophalangeal synovial membrane lesions in patients with rheumatoid arthritis(RA).MethodsA total of 1540 MCP joints in 154 untreated early RA patients underwent ultrasonography.Select the synovial hyperplasia of MCP joints for contrast-enhanced ultrasound examination,Comparison of two-dimensional images and ultrasound synovial thickness,To analyze the correlation between changes of synovial thickness and energy Doppler flow classification during contrast echocardiography.Results There were 431(431/1540,28%)MCP appeared different degrees of synovial hyperplasia,the dorsal synovial hyperplasia 319(74%),Palmar synovial thickening 77(17.9%),bilateral synovial thickening of 35(8.1%);The thickness of synovial membrane measured by two-dimensional images was 2.3±1.6.The thickness of synovial membrane measured by echocardiography was 2.7±2.1.Conclusions The use of ultra-high frequency intraoperative ultrasound probe combined with contrast-enhanced ultrasound in the diagnosis of metacarpophalamic synovial hyperplasia in patients with early RA significantly increased the sensitivity,measurement error is reduced.
