实时超声弹性成像技术结合声学密度定量技术对颈动脉斑块稳定性的研究

【摘要】 目的应用实时超声弹性成像技术结合声学密度定量(AD)技术分析颈总动脉粥样硬化斑块的弹性图像及声学密度的相关性。方法对76例颈动脉粥样硬化患者共122个斑块进行实时弹性成像及AD检测,计算低回声斑块、混合回声斑块、强回声斑块的应变值的比值(SR比值)与背向散射积分校正值(IBS%值),分析不同回声斑块的SR比值与IBS%值及相关性,分析SR比值、IBS%值与脑梗死的发病率的关系。结果低回声斑块组、混合回声斑块组、强回声斑块组的SR比值均数分别为(2.47±0.93)、(4.71±0.89)、(6.15±0.70),三组之间均数的差异具有统计学意义(F=196,P<0.001);低回声斑块组、混合回声斑块组、强回声斑块组的IBS%值均数分别为(3.02±0.68)、(5.01±1.10)、(8.86±1.80),三组之间均数的差异具有统计学意义(F=223,P<0.001)。斑块的不同回声类型与SR比值及IBS%值呈显著性正相关(P<0.001)。颈动脉斑块SR比值<5的患者脑梗死的发病率为63.83%,>5的患者为24.14%,两者之间的差异有统计学意义(χ2=11.31,P<0.001);颈动脉斑块IBS%值<4的患者脑梗死的发病率为65.91%,IBS%值>4的患者为25%,两者之间的差异有统计学意义(χ2=12.41,P<0.001)。结论综合利用超声弹性成像技术及AD技术可以对斑块的应变值及回声强度两方面进行综合定量分析,从而更准确地评价斑块的稳定性,预测脑梗死发病的危险性。

【Abstract】 Objective To analyze the relationship between the elastography images and the acoustic density of carotid atherosclerotic plaques by real-time ultrasonic elastography combined with acoustic densitometry.Methods 122 plaques in 76 patients with carotid atherosclerosis were examined with the real-time ultrasonic elastography and acoustic densitometry.The SR ratio and IBS% value of hypoechoic plaques,hybrid plaques and hyperechoic plaques were calculated.The SR ratio,IBS% value of different echo plaque and correlation of them were analyzed.The relationship between SR ratio,IBS% value and the incidence of cerebral infarction was analyzed.Results The SR ratio and IBS%value of hypoechoic plaques,hybrid plaques and hyperechoic plaques group were increased successively.There were significant positive correlations among the different echo types of plaques,SR ratio and IBS% value.The incidence of cerebral infarction in patients with carotid plaques with SR<5 and IBS%<4 were significantly higher than that of SR ratio>5 and IBS% >4.The difference was statistically significant.Conclusions The strain values and the echo intensity of plaques could be analyzed comprehensively and quantitatively by ultrasonic elastography combined with acoustic densitometry.Therefore,the stability of plaques was evaluated more accurately.The risk of cerebral infarction could be predicted.
