二维斑点追踪技术评价系统性红斑狼疮并发不同程度肺动脉收缩压患者右心室功能
Assessment of Right Ventricular Function in Systemic Lupus Erythematosus Patients with Different Pulmonary Artery Systolic Pressure by Two-dimensional Speckle Tracking Echocardiography
【摘要】 目的通过二维斑点追踪技术(2D-STE)评价系统性红斑狼疮(SLE)并发不同程度肺动脉收缩压(PASP)患者右心室功能的变化。方法选取A组(对照组)为健康志愿者35例。根据患者PASP的不同将109例SLE患者分为3组(实验组),B组PASP≤30mm Hg,C组30mm Hg<PASP<50mm Hg,D组PASP≥50mm Hg。2D-STE测量参数包括:心尖四腔切面右心室游离壁基底段、中间段及心尖段收缩期纵向峰值应变(ε)、收缩期峰值应变率(SRs)、舒张早期峰值应变率(SRe)、舒张晚期峰值应变率(SRa)。结果与A组及B组相比,C、D组的右室游离壁各节段ε、SRs、SRe、SRa均减低,差异有统计学意义(P<0.05)。C、D组相对应节段ε出现递减趋势,差异有统计学意义(P<0.05)。C、D组相对应节段SRs、SRe、SRa,组间差异不明显,差异无统计学意义(P>0.05)。结论 2D-STE可作为评价不同程度PASP的SLE患者右心室功能的有效方法。
【Abstract】 Objective To evaluate the right ventricular function in patients with systemic lupus erythematosus(SLE)with different pulmonary artery systolic pressure(PASP)by two-dimensional speckle tracking echocardiogrhaphy(2 D-STE).Methods Thirty-five matched healthy adults selected as the control group A.In addition,109 patients with SLE were divided into three groups according to the difference of pulmonary artery systolic pressure(PASP),group B was PASP≤30 mm Hg,30 mm Hg <PASP<50 mm Hg as group C,group D was PASP≥50 mm Hg.Longitudinal peak systolic strain(ε)and strain rate(SRs),early diastolic strain rate(SRe)and late diastolic strain rate(SRa)were measured in right ventricular free wall for basal,mid and apical segment in patients with SLE and healthy controls by 2 D-STE from the apical four-chamber view.Results Compared with group B and group A,ε,SRs,SRe and SRa were obviously decreased in group C and group D(P<0.05).εof all segments in group C and group D were reduced progressively(P<0.05).The difference of SRs,SRe and SRa between group C and group D was not obvious(P>0.05).Conclusions 2 D-STE can be used as an effective method for evaluating right ventricular function in SLE patients with different PASP.
