彩色多普勒超声评估胎心时期心血管血流动力学特点分析
Analysis on cardiovascular hemodynamic characteristics in fetal heart period evaluated by color Doppler ultrasonography
【摘要】 目的探讨彩色多普勒超声评估胎心时期心血管血流动力学特点,为不同孕周胎儿的心血管发育情况进行个体化管理提供参考。方法将2015年1月-2017年1月在宜宾市第一人民医院产检的120例孕周为16~42周胎儿作为研究对象,根据孕周的不同将孕16~25周31例胎儿分为A组、孕26~30周29例分为B组、孕31~35周28例分为C组、孕36~42周32例分为D组。所有胎儿均行彩色多普勒超声检查,对胎儿胎心时期二尖瓣、三尖瓣反流及主动脉、肺动脉导管血流动力学特点进行综合评价。结果 4组胎儿经彩色多普勒超声对二尖瓣、三尖瓣、主动脉、肺动脉的血流显像成功率分别为90.83%、93.33%、90.00%、49.17%,其心血管血流在速度、方向、分布等方面与正常儿童及成人呈现出明显的差异性。结论彩色多普勒超声能够对胎心时期血流动力学予以评估,在临床诊断期间需加强对胎儿心血管血流动力学特点的关注。
【Abstract】 Objective To explore cardiovascular hemodynamic characteristics in fetal heart period evaluated by color Doppler ultrasonography,provide reference for individualized management of cardiovascular development of fetuses at different gestational weeks. Methods From January 2015 to January 2017,120 fetuses during 16-42 gestational weeks receiving prenatal examination in the First People’s Hospital of Yibin were selected and divided into group A( 16-25 gestational weeks),group B( 26-30 gestational weeks),group C( 31-35 gestational weeks),and group D( 36-42 gestational weeks). Color Doppler ultrasonography was performed,regurgitation of valvula bicuspidalis and tricuspid valve, hemodynamic characteristics of aorta and pulmonary artery catheter were evaluated comprehensively.Results In the four groups,the successful rates of blood flow imaging of valvula bicuspidalis,tricuspid valve,aorta,and pulmonary artery by color Doppler ultrasonography were 90. 83%,93. 33%,90. 00%,and 49. 17%,respectively. Cardiovascular blood flow showed significant differences in velocity,direction,and distribution compared with normal children and adults. Conclusion Color Doppler ultrasound can assess hemodynamics in fetal heart period. During clinical diagnosis,more attention should be paid to fetal cardiovascular hemodynamic characteristics.
