18F-FDG-PET-CT在软组织转移瘤诊断中的应用
Application of 18F-FDG-PET-CT in diagnosis of soft tissue metastases
【摘要】 目的探讨18F-FDG-PET-CT显像在软组织转移瘤诊断中的临床应用价值。方法选取2011年7月2017年8月经广东省中医院PET/CT诊断软组织转移瘤患者69例。观察转移灶的数目、部位、形态、大小、密度,并计算最大标准摄取值(SUVmax)。计算肌肉及皮下软组织转移灶PET/CT显示率。结果 (1)肌肉转移灶:PET/CT显示100个;PET显示100个、显示率为100%,SUVmax:1.9115.93;CT平扫显示肌肉转移灶32个、显示率为32%;分为三型:结节、肿块型,炎症型,钙化型。(2)肌肉转移灶CT增强(16例):同扫描范围PET/CT显示26个,CT增强显示11个(显示率为42.3%),表现为均匀、不均匀或环形轻度明显强化。(3)皮下软组织内转移灶:PET/CT显示78个;CT显示78个,显示率为100%,表现为多发或单发结节或肿块;PET显示74个、显示率为94.9%,SUVmax:1.1311.73。(4)肌间隙软组织内转移灶:PET/CT显示7个,表现为单发结节,SUVmax:1.266.42。结论 PET/CT不但能全面显示软组织转移瘤的形态、部位及代谢等情况,还能明确原发肿瘤,在软组织转移瘤的诊断与鉴别诊断中优于单独PET或CT,具有独特的优势。
【Abstract】 Objective To investigate the clinical value of 18F-FDG-PET-CT imaging in the diagnosis of soft tissue metastases. Methods 69 cases of soft tissue metastases diagnosed by PET/CT in Guangdong Provincial Chinese Medicine Hospital from March 2011 to August 2017 were selected. The number, location, shape, size and density of the metastases were observed and the maximum standard uptake value(SUVmax) was calculated. The PET/CT display rate of muscle and subcutaneous soft tissue metastases was calculated. Results(1) Muscle metastases: PET/CT showed 100 and PET showed 100, with the display rate of 100%, and the SUVmax was 1.9115.93; CT plain scan showed 32 muscle metastases, with the display rate of 32%; the muscle metastases were divided into three types: nodules/masses, inflammation and calcifications.(2) CT enhancement of muscle metastases(16 cases): there were 26 cases in the same scan range PET/CT and 11 in CT enhancement(display rate of 42.3%), with the performance of uniform, non-uniform or slightly mild to obvious enhancement.(3) Metastatic lesions in the subcutaneous soft tissue: 78 on PET/CT; 78 on CT,with a display rate of 100%, with multiple or single nodules or lumps; 74 with PET, and the display rate was 94.9%,SUVmax:1.13 to 11.73.(4) Intramuscular soft tissue metastasis: PET/CT showed 7 with single nodule, and the SUVmax:1.26 6.42. Conclusion PET/CT not only can fully display the morphology, location and metabolism of soft tissue metastases, but also can identify the primary tumors, which is superior to PET or CT alone in the diagnosis and differential diagnosis of soft tissue metastases, and has unique advantages.
