【摘要】 目的:了解福建省立医院(以下简称"我院")静注人免疫球蛋白(pH4)临床使用情况及合理性,针对不合理现象提出改进措施,使其临床应用更合理、规范。方法:采用回顾性分析方法,选择我院2016年使用静注人免疫球蛋白(pH4)的596例患者,对患者性别、年龄及疾病诊断等信息进行统计分析;以国外相关临床指南为基础,结合药品说明书,对我院静注人免疫球蛋白(pH4)的临床应用合理性进行评价。结果:有效病例为595例,其中男性患者341例,女性患者254例;年龄0～100岁,平均32.2岁,以<10岁儿童居多;我院共有34个科室使用静注人免疫球蛋白(pH4),主要集中于儿科、重症医学等科室,常用于重症肺炎、新生儿ABO溶血症等疾病;超说明书用药168例(占28.2%),指南不推荐或未提及用药病例89例(占15.0%)。结论:我院静注人免疫球蛋白(pH4)存在超说明书使用情况,医院相关部门应加强用药监管,促进合理用药。更多还原

【Abstract】 OBJECTIVE: To investigate the clinical application rationality of human immunoglobulin(pH4) for intravenous injection in Fujian Provincial Hospital(hereinafter referred to as "our hospital"), and to put forward improvement measures for irrational phenomenon, so as to to make the clinical application more reasonable and standardized. METHODS: Retrospective analysis was adopted, 596 patients with application of human immunoglobulin(pH4) for intravenous injection were extracted from our hospital in 2016, and statistical analysis was conducted on patients’ gender, age and disease diagnosis. Based on the relevant clinical guidelines abroad, combined with the drug instructions, the clinical application rationality of human immunoglobulin(pH4) for intravenous injection in our hospital was evaluated. RESULTS: The effective cases were 595, including 341 males and 254 females, aged 0-100 years old, with an average of 32.2 years old, and children <10 years took the lead. A total of 34 departments in our hospital applied human immunoglobulin(pH4) for intravenous injection, mainly concentrated in pediatrics, critical care and other departments, often used in severe pneumonia, neonatal ABO hemolysis and other diseases. There were 168 cases(28.2%) with off-label drug use, and 89 cases(15.0%) were not recommended or not mentioned in the guidelines. CONCLUSIONS: In our hospital, there is off-label drug use in human immunoglobulin(pH4) for intravenous injection, therefore, relevant departments of hospitals should strengthen the drug administration supervision and promote the rational drug use.更多还原
