呼吸内科支气管肺泡灌洗液非发酵菌临床观察
Clinical observation of nonfermenting bacteria in respiratory bronchoalveolar lavage fluid
【摘要】 目的:分析某院呼吸内科患者支气管肺泡灌洗液非发酵菌种类及耐药性,为临床诊治和合理使用抗菌药物提供参考。方法:收集某院2014年1月—2016年12月呼吸内科肺部感染患者的支气管肺泡灌洗液标本,对其分离的非发酵菌进行回顾性分析。结果:1454名患者BALF标本共分离出189株非发酵菌,主要为鲍曼不动杆菌、铜绿假单胞菌及嗜麦芽窄食单胞菌。结论:呼吸内科肺部感染患者支气管肺泡灌洗液分离出的非发酵菌以鲍曼不动杆菌为主,临床医生应该根据药敏实验结果合理的使用药物。
【Abstract】 Objective To analyze the types and drug resistance of non-fermentative bacteria in bronchoalveolar lavage fluid in a hospital respiratory medicine,For clinical diagnosis and rational use of antimicrobial agents provide a reference.Methods The specimens of bronchoalveolar lavage fluid from patients with respiratory infection in lung hospital from January2014 to December 2016 in a hospital were collected and their non-fermentative bacteria were retrospectively analyzed.Results A total of 189 strains of non-fermentative bacteria were isolated from 1454 BALF specimens, mainly Acinetobacter baumannii, Pseudomonas aeruginosa and Stenotrophomonas maltophilia, accounting for 54.5%, 29.6%, 6.8%, respectively The results showed that the drug resistance rates of Acinetobacter baumannii to amikacin and cefoperazone/sulbactam were less than 50%, and the rates of resistance to other antibiotics were more than 70%. Pseudomonas aeruginosa pairs Imipenem resistant rate of more than 70%. Conclusion Acinetobacter baumannii is the main non-fermentative bacterium isolated from bronchoalveolar lavage fluid in patients with pulmonary infection in respiratory medicine. The drug resistance of the non-fermentive bacteria is at a relatively high level. Clinicians should use drugs reasonably according to the results of drug susceptibility testing.
