血清Glypican 4对胰岛素抵抗和胰岛β细胞功能的影响

【摘要】 目的 探讨GPC4与2型糖尿病胰岛素敏感性、胰岛β细胞功能及体脂的相关性。方法 纳入2013年1月~2016年12月在武汉大学中南医院住院并完成胰岛素释放试验的患者329例。按体质指数分为正常组、超重组和肥胖组。对所有患者进行糖耐量试验、人体测量学、血脂谱和GPC4检测。胰岛细胞功能应用总时相胰岛素分泌指数(AUCins/gluc120)、胰岛素敏感性应用HOMA-IR进行评估。结果 超重组和肥胖组的GPC4水平明显高于正常组(P<0.001);GPC4与2型糖尿病患者的HOMA-IR水平呈明显正相关(r=0.177,P<0.05)。四分位组分析显示:随着HOMA-IR水平的升高,GPC4水平呈逐渐升高趋势(P<0.05),随着AUCins/gluc120水平的升高,GPC4水平呈逐渐上升趋势,但没有统计学差异(P均>0.05)。结论 GPC4与2型糖尿病患者的肥胖程度和胰岛素抵抗程度明显相关,是2型糖尿病治疗的潜在靶点。

【Abstract】 Objective Glypican4( GPC4) is a recently identified adipokine. This research is set to explore the relationship between GPC4 and insulin sensitivity, beta cell function and obesity in type 2 diabetic patients. Methods Three hundred and twenty-nine in-patients of Zhongnan Hospital between Jan 2013 to Dec 2016 were recruited in this study. Among them,there are 187 males and 142 females,average age were 43. 22 ± 11. 54 years old. All patients undertook examinations on OGTT,anthropometric indexes,lipid profile and GPC4. AUCins/gluc120 was used to evaluate islet beta cell function. HOMA-IR was used to evaluate insulin sensitivity. Results GPC4 is significantly higher in overweight and obesity group compared to control(P < 0. 001). GPC4 is positively correlated to HOMA-IR in type 2 diabetic patients( r =0. 177,P <0.05). Quartile analysis of HOMA-IR showed that GPC4 level will gradually increase with the increase of HOMA-IR(P < 0. 05). Quartile analysis of AUCins/gluc120 showed that GPC4 level will gradually increase with the increase of AUCins/gluc120,but the trend is not statistically significant( P >0. 05). Conclusion GPC4 is related to obesity and insulin resistance in type 2 diabetes and will be a potential target for diabetic treatment.
