等速肌力训练模式干预在老年脑卒中偏瘫步行功能康复中的作用探讨

【摘要】 目的探讨等速训练肌力模式干预对老年脑卒中偏瘫患者步行功能康复的作用。方法将180例老年脑卒中偏瘫康复期患者随机分为观察组和对照组各90例,对照组采用一般康复训练,观察组在一般康复训练的同时增加偏瘫下肢伸、屈膝肌等速肌力训练。治疗前和治疗6周后评定2组步行能力(站起走计时测试TΜG)、膝关节等速肌力测试偏瘫侧峰力矩(PT)、日常生活活动能力(ADL)、肌张力(MAS)。结果治疗6周后,2组患者步行能力均较治疗前明显提高,站起走计时测试时间观察组短于对照组(P<0.05)。2组患者膝关节伸屈膝肌PT、ADL评分均较治疗前明显提高,且观察组显著优于对照组(P<0.05)。治疗前后2组患者肌张力MAS评分比较差异无统计学意义(P>0.05)。结论老年脑卒中偏瘫患者应用等速肌力训练模式干预可明显提高步行功能。

【Abstract】 Objective To study effects of isokinetic exercise intervention model on walking ability recovery of the aged stroke hemiplegic patients. Methods A total of 180 patients with hemiplegia in the aged stroke were random Ly divided into two groups :experimental group,90 cases and control group,90 cases. All the cases acceped routine rehabilitation exercise,The experimental group additionally accepted isokinetic exercise for knee flexion&extension muscles of the hemiplegic side.In before and the six weeks after treating,their lower limb function was measured by walking ability(Timed up and go test T ΜG),isokinetic peak torpue(PT), activities of daily living(ADL)and Modified Ashworth score(MAS). Results The six weeks after treating,walking function,isokinetic peak torpue of knee muscles and ADL recovery in two groups were increased significantly as compared with those pretreatment(P<0.05). All the scores in experimental group were obviously superior to those of control group(P<0.05).Modified Ashworth score showed no significant difference between experimental group and control group.Conclusion Isokinetic exercise intervention model can promote to the recovery of walking ability in the aged stroke hemiplegic patients.
