肥胖相关检测指标对血糖异常筛查和预测价值的比较

【摘要】 目的:本文的目的在于对既往评价肥胖测量指标预测糖尿病价值的相关研究进行归纳、总结,通过系统综述的方法比较不同肥胖测量指标对糖尿病的预测价值,选择一种简单的肥胖测量指标来作为中国成人2型糖尿病的筛查方法。方法:在Pubmed数据库和万方医学数据库检索相关文献,参考Cochrane协作网操作手册的评价方法对文献进行筛选、归纳、总结。结果:共筛选出39篇文献,中文12篇,英文27篇,共纳入研究对象764269例。在横断面研究中,通过受试者工作特征曲线下面积(area under receiver operating characteristic curve,AUROC)和比值比(odds ratio,OR)值两种方法对各项肥胖指标进行评价,均表明腹型肥胖指标对2型糖尿病的筛查价值高于体质指数(body mass index,BMI);在队列研究中,腹型肥胖指标对2型糖尿病的预测价值并不优于BMI。结论:在中国人群中,腰围身高比(waist-to-height ratio,WHtR)、腰围(waist circumference,WC)和腰臀比(waist-to-hip ratio,WHR)作为腹型肥胖测量指标对于2型糖尿病的筛查价值优于BMI,其中最佳测量指标为WHtR,诊断切点宜选择0.5。

【Abstract】 Objective: The purpose of this paper is compared different measurements of obesity on screening for type 2 diabetes mellitus in Chinese adults, through systematic review methods, then select a simple measurement of obesity as a method of screening T2DM. Methods: Searching the related literatures on Pubmed and Wanfang medical databases, and evaluating papers according to the Cochrane Collaboration network operation manual. Results: A total of 39 articles were chosen in the study, including 12 Chinese articles and 27 English articles, in a sum of 764269 cases. In cross-sectional studies, AUROC and OR were used to evaluate the obesity index, showing that abdominal obesity indices for screening T2DM were better than BMI, in cohort studies, abdominal obesity indices for prediction of type 2 diabetes were not different with BMI significantly. Conclusion: In the Chinese population, waist height ratio, waist circumference and waist-hip ratio as indicators of abdominal obesity are more valuable in screening for type 2 diabetes than body mass index; of which waist height ratio is the best measurement, and 0.5 should be chosen as a cutoff point.
