血清淀粉样蛋白A及CD64在新生儿败血症诊断中的价值

【摘要】 目的:研究血清淀粉样蛋白A(SAA)及中性粒细胞CD64早期临床诊断新生儿败血症及疗效评价的意义。方法:选择福建医科大学附属福州市第一医院2016年3月-2017年9月收治的新生儿败血症患儿56例为观察组,选择同期非感染新生儿42例为对照组。采用胶体金法和流式细胞分析术分别检测SAA和CD64,观察这两项指标对于新生儿败血症诊断的特异性、敏感性、阴性、阳性预测值及约登指数。结果:观察组治疗前SAA、CD64值明显升高,与治疗后、对照组比较,差异均有统计学意义(P<0.05),联合早期诊断新生儿败血症优于单个指标独立诊断。结论:SAA和CD64联合测定在新生儿败血症早期诊断具有较高的敏感性及特异性,同时有助于判断疗效及预后。

【Abstract】 Objective:To evaluate the therapeutic effects of serum amyloid A(SAA) and neutrophil CD64 in early diagnosis of neonatal sepsis.Method:Fifty-six cases of neonatal sepsis admitted to the Fuzhou First Hospital Affiliated to Fujian Medical University from March 2016 to September 2017 were selected as the observation group,and 42 non-infected neonates were selected as the control group.The DIGFA method and flow cytometry(FACS) were used to detect levels of SAA and CD64,respectively.The specificity,sensitivity,negative,positive predictive value,and the Youden index in both groups were compared.Result:Before treatment,SAA and CD64 levels in the observation group were significantly increased compared with the control group,and the differences were statistically significant(P<0.05).It is believed that the early diagnosis of neonatal sepsis with multiple indicators was better than that of single indicator.Conclusion:The combined determination of SAA and CD64 levels has higher sensitivity and specificity in the early diagnosis of neonatal sepsis,which is helpful to evaluate the efficacy and prognosis.
