牵引器辅助复位微创内固定系统治疗股骨髁复杂骨折的临床效果

【摘要】 目的探讨应用骨折牵引器辅助复位微创内固定系统(less invasive stabilization system,LISS)治疗复杂股骨远端骨折临床效果与影响因素。方法 2015年—2017年应用骨折牵引器辅助复位微创内固定系统治疗复杂股骨远端骨折,男13例,女5例;均采用髌骨旁外侧切口,需辅助髌骨旁内侧切口3例,单纯应用LISS接骨板固定骨折14例,应用LISS接骨板结合空心螺钉或松质骨复位固定骨折4例。结果所有患者术后均获得随访,随访时间为10~24个月,平均18个月。骨折愈合时间平均(8.10±3.40)个月,无骨不连发生,术中无植骨,术后无一例发生切口感染、内固定物松动及断裂。结论微创内固定系统治疗成人股骨髁复杂骨折是一种理想的内固定技术,双反牵引器辅助骨折复位可使复杂股骨髁骨折复位简单化,可以术中快速复位,使手术操作相对简单,为微创操作提供有利帮助,并为术后膝关节功能恢复及促进骨折愈合提供有力帮助。

【Abstract】 Objective To investigate clinical effects and influencing factors of the less invasive stabilization system(LISS)assisted by fracture traction reduction apparatus in treatment of complex distal femoral condylar fractures. Methods From 2015 to 2017,13 males and 5 females with complex distal femoral condylar fractures were treated with LISS assisted by fracture traction reduction apparatus.All cases were treated with lateral parapatellar incision,and 3 cases needed medial parapatellar incision supplementation.Fourteen cases were fixed with LISS interal fixation plate alone for fracture fixation,and 4 cases were treated with LISS plate combined with hollow screw or cancellous bone reduction for fracture fixation. Results All patients were followed up for 10~24 months(average:18 months).The average healing time was(8.10±3.40)months in all patients.There was no bone nonunion,no intraoperative bone grafting,no postoperative incision infection and no loosening and breaking of internal fixation. Conclusion LISS is an ideal internal fixation technique for the treatment of adult complex femoral condylar fractures.Double reverse traction apparatus assisted fracture reduction can simplify the reduction of complex femoral condylar fracture,facilitate rapid intraoperative reduction and make the operation relatively simple.It provides favorable help for minimally invasive operation,as well as for postoperative recovery of knee joint function and for promoting of fracture healing.
