【摘要】 目的探讨联合检测胃蛋白酶原(PG)、胃泌素17 (G-17)及幽门螺旋杆菌(HP)感染对胃癌前病变及胃癌的诊断价值。方法随机选择2016年6月至2017年12月因胃部不适于我院就诊的患者,根据内镜及病理结果将患者分为胃良性病变组、胃癌前病变组以及胃癌组,比较三组患者的PGⅠ、 PGⅡ、 PGR、 G-17水平及HP感染率,计算单独及联合检测的阳性率。结果胃良性病变组的PGⅠ及PGR均显著高于胃癌前病变组及胃癌组(P <0.05), PGⅡ及G-17显著低于胃癌组(P <0.05);胃癌前病变组的G-17显著低于胃癌组(P <0.05)。单项检测PGⅠ、 PGR、 HP及联合检测的阳性率在胃良性病变组、胃癌前病变组及胃癌组中逐步升高。结论联合检测PG、 G-17、 HP可帮助临床筛查胃癌前病变及胃癌,具有重要的临床价值。更多还原

【Abstract】 Objective To explore the diagnostic value of the joint detection of pepsinogen(PG), gastrin 17(G-17) and helicobacter pylori(HP) infection for gastric precancerous lesions and gastric cancer. Methods Patients with gastric discomfort admitted to our hospital from June 2016 to December 2017 were randomly selected. All cases were divided into benign gastric lesion group, gastric precancerous lesion group and gastric cancer group according to the endoscopic and pathological findings. The levels of PGⅠ, PGⅡ, PGR and G-17, and HP infection rate were compared among the three groups, and the positive rates of single detection and joint detection were calculated.Results The PGⅠ and PGR of the benign gastric lesion group were significantly higher than those of the gastric precancerous lesion group and gastric cancer group(P <0.05), and the PGⅡ and G-17 were significantly lower than those of the gastric cancer group(P <0.05); The G-17 of the gastric precancerous lesion group was significantly lower than that of the gastric cancer group(P <0.05). The positive rates of PGⅠ, PGR, HP and joint detection were increased gradually in the benign gastric lesion group, gastric precancerous lesion group and gastric cancer group. Conclusions The joint detection of PG, G-17 and HP is helpful in the screening of gastric precancerous lesions and gastric cancer, which has important clinical value.更多还原
