【摘要】 目的观察腔内超声诊断剖宫产术后子宫下段瘢痕妊娠的临床价值。方法选择80例疑似为子宫下段瘢痕妊娠的孕妇为研究对象,所有孕妇均行经腹部彩色多普勒超声检查和腔内彩色多普勒超声检查。以随诊结果为最终诊断结果,观察腔内超声和经腹部超声对子宫下段瘢痕妊娠的诊断准确率、误诊率和漏诊率,观察腔内超声诊断的灵敏度和特异度。结果以随访结果作为最终诊断结果, 80例疑似瘢痕妊娠患者中76例为瘢痕妊娠,腔内超声诊断的灵敏度为97.37%(74/76),特异度为75.00%(3/4)。腔内超声对子宫下段瘢痕妊娠的诊断准确率为92.50%,显著高于经腹部超声的81.25%(P <0.05);腔内超声与经腹部超声诊断的误诊率和漏诊率无统计学差异(P>0.05)。结论腔内超声对于剖宫产术后子宫下段瘢痕妊娠具有重要的临床诊断价值。更多还原

【Abstract】 Objective To observe the clinical value of endoluminal ultrasound in the diagnosis of scar pregnancy in lower uterus after cesarean section. Methods 80 cases of pregnant women with suspected scar pregnancy in lower uterus were selected as research objects. All the cases received transabdominal color Doppler ultrasound and endoluminal color Doppler ultrasound. The follow-up results were used as the final diagnosis results, to observe the accuracy, misdiagnosis rate and missed diagnosis rate of transabdominal ultrasound and endoluminal ultrasound in the diagnosis of scar pregnancy in lower uterus, and to observe the sensitivity and specificity of endoluminal ultrasound.Results With follow-up results as the final diagnosis results, 76 cases were scar pregnancy among 80 patients with suspected scar pregnancy,and the sensitivity and specificity of endoluminal ultrasound were 97.37%(74/76) and 75.00%(3/4) respectively. The accuracy of endoluminal ultrasound in the diagnosis of scar pregnancy in lower uterus was 92.50%, significantly higher than 81.25% of transabdominal ultrasound(P<0.05); No statistical difference was found between the endoluminal ultrasound and the transabdominal ultrasound in the misdiagnosis rate or missed diagnosis rate(P >0.05).
