【摘要】 目的分析全髋关节置换术治疗股骨头坏死(ONFH)和髋关节发育不良(DDH)的疗效。方法选取2015年9月至2018年1月我院收治的40例ONFH患者作为ONFH组,同期选取40例CroweⅠ型、Ⅱ型DDH患者作为DDH组。两组患者均采用生物型全髋置换术治疗,分析比较两组患者的围术期相关指标、手术前后Harris评分以及并发症发生率。结果与DDH组比较, ONFH组的术中出血量较少,住院时间和首次下床行走时间较短,双下肢长度差异较小(P均<0.05)。术后1个月、 2个月、 3个月,两组患者的Harris评分均显著高于术前,且ONFH组的Harris评分高于DDH组(P均<0.05)。ONFH组的并发症发生率低于DDH组(P <0.05)。结论全髋关节置换术治疗ONFH和DDH均可明显改善患者的髋关节功能,促进其疾病转归,及早恢复正常生活。相对于ONFH,全髋关节置换术治疗DDH的并发症较多,恢复时间较长。更多还原

【Abstract】 Objective To analyze the effect of total hip arthroplasty in the treatment of osteonecrosis of the femoral head(ONFH) and developmental dysplasia of the hip(DDH). Mehods 40 cases of ONFH patients admitted to our hospital from September 2015 to January2018 were selected as ONFH group. In the same period, 40 cases of patients with Crowe Ⅰ and Ⅱ DDH were selected as DDH group. Both groups were treated with biologic total hip arthroplasty. The perioperative related indicators, Harris score before and after operation, and incidence of complications were analyzed and compared between the two groups. Results Compared with DDH group, The intraoperative blood loss of ONFH group was less, the hospitalization time and first time to walk out of bed were shorter, and the length difference of lower limbs was less(all P <0.05). 1 month, 2 months and 3 months after operation, the Harris scores of both groups were significantly higher than those of DDH group, and the Harris scores of ONFH group were higher than those of DDH group(all P <0.05). The incidence of complications of ONFH group was lower than that of DDH group(P <0.05). Conclusions Total hip arthroplasty in the treatment of ONFH and DDH can significantly improve the hip joint function, promote patients’ disease outcome, and return to normal life early. Compared with ONFH, total hip arthroplasty has relatively more complications and longer recovery time for DDH.更多还原
