乳管镜辅助手术治疗乳腺导管内乳头状肿瘤的临床效果观察

【摘要】 目的评价纤维乳管镜(FDS)辅助手术治疗乳腺导管内乳头状肿瘤(IDPN)的临床效果。方法选取2015年8月至2017年8月与我院收治的IDPN患者78例,根据治疗方法不同分为常规组和乳管镜组各39例。常规组行部分腺体切除术治疗,乳管镜组在FDS引导及定位下行IDPN切除术治疗。比较两组患者的治疗效果。结果与常规组相比,乳管镜组的切口长度较短,切除标本重量较少,但手术时间明显较长(P均<0.05)。乳管镜组的病理检出率为94.87%,显著高于常规组的64.10%(P<0.05)。乳管镜组的并发症发生率为0.00%,明显低于常规组的17.95%(P <0.05)。结论 FDS辅助手术治疗IDPN具有定位准确、手术切口小、腺体破坏少、术后美容效果好、并发症少以及病理检出率高等优势,值得临床推广。

【Abstract】 Objective To evaluate the clinical effect of fiberoptic ductoscopy(FDS) assisted surgery in the treatment of intraductal papillary neoplasms(IDPN). Methods 78 cases of IDPN patients admitted to our hospital from August 2015 to August 2017 were selected and divided into routine group and ductoscopic group according to the different treatment methods, with 39 cases in each group. The routine group received partial mastectomy, while the ductoscopic group received IDPN resection guided by FDS. The treatment effects were compared between the two groups. Results Compared with the routine group, the incision length of the ductoscopic group was shorter, the resection specimen weight was less, but the operation time was significantly longer(all P <0.05). The pathological detection rate of the ductoscopic group was 94.87%, significantly higher than 64.10% of the routine group(P <0.05). The incidence of complications of the ductoscopic group was 0.00%, significantly lower than 17.95% of the routine group(P <0.05). Conclusions FDS assisted surgery in the treatment of IDPN has the advantages of accurate positioning, small incision, less damage of breast, good cosmetic effect, less complications,and higher pathological detection rate, which is worthy of clinical promotion.
