【摘要】 目的探讨2017年欧洲甲状腺协会成人甲状腺结节超声恶性危险分层指南(EU-TIRADS)的临床应用价值。方法回顾性分析301例甲状腺结节患者446个结节的超声图像,根据EU-TIRADS对其分类并与术后病理结果对照,绘制ROC曲线,计算其敏感性、特异性、阳性预测值、阴性预测值,准确性。结果 446个甲状腺结节术后病理诊断:良性结节325个(72.9%),恶性结节121个(27.1%)。取EU-TIRADS 5时为阳性、其余分类诊断为阴性时,EU-TIRADS诊断甲状腺结节的曲线下面积最大(0.906),此时敏感性、特异性、阳性预测值、阴性预测值、准确性分别为95.9%、83.7%、68.6%、98.2%、87.0%。结论 EU-TIRADS能较准确地评估甲状腺结节的恶性风险,为临床诊疗提供指导依据。更多还原

【Abstract】 Objective The purpose of this study was to explore the clinical value of European Thyroid Association guidelines for ultrasound malignancy risk stratification of thyroid nodules in adults(the EU-TIRADS). Methods Ultrasound images of 446 thyroid nodules in 301 patients who were confirmed by pathology were classified according to the EU-TIRADS. ROC curves were constructed to calculate the sensitivity,specificity,positive/negative predictive values and accruacy of EU-TIRADS. Results Of the 446 nodules,325 nodules were benign and 121 nodules were malignant. Best cut-off point for diagnosing malignant by the EUTIRADS were EU-TIRADS 5. The areas of curve,sensitivity,specificity,positive/negative predictive values and accuracy of EUTIRADS were 0. 906%,95. 9%,83. 7%,68. 6%,98. 2%,87. 0% at that point. Conclusion The EU-TIRADS may provide an appropriate method for estimating risk of malignancy for clinical management of thyroid nodules.更多还原
