双源CT真实平扫与虚拟平扫对孤立性肺结节的鉴别诊断价值
Differential diagnosis of benign and malignant solitary pulmonary nodules by real scanning and dual-source virtual scanning in CT
【摘要】 目的探讨双源CT真实平扫与虚拟平扫对良恶性孤立性肺结节的鉴别诊断价值。方法将本院收治的53例孤立性肺结节患者作为研究对象,均采取CT常规平扫和双能增强扫描。比较虚拟平扫和真实平扫图像结节直径、噪声、CT值、信噪比、钙化检出率、碘图CT值、增强强化值及诊断准确性等情况。结果两种平扫图像所示结节直径、噪声、CT值和信噪比的比较,均无明显差异(P>0.05)。虚拟平扫图像检出淋巴结钙化86个,其检出率为93.48%(86/92)。虚拟平扫和真实平扫碘图CT值的比较,并无明显差异(P>0.05);虚拟平扫的增强强化值较真实平扫显著升高(P<0.05);虚拟平扫和真实平扫的碘图CT值较增强强化值均显著升高(P<0.01)。碘图CT值诊断灵敏度较常规平扫和虚拟平扫显著升高(P<0.05);虚拟平扫诊断的特异度较碘图CT值、常规平扫均显著升高(P<0.05);碘图CT值诊断的准确度稍高于常规平扫和虚拟平扫,但并无显著差异(P>0.05)。结论双源CT虚拟平扫对良恶性孤立性肺结节具有良好的临床鉴别诊断价值。
【Abstract】 Objective To analyze the differential diagnostic value of benign and malignant solitary pulmonary nodules by dualsource CT for real and virtual scanning. Methods 53 patients with solitary pulmonary nodules admitted to our hospital as the research objects were performed the CT routine scan and dynamic enhanced scanning. The nodule diameter,CT value,signal to noise ratios,the detection rate of calcification,iodine figure CT value,enhancement value,accuracy etc. by real and virtual scanning image were compared. Results The scanning image of the two methods showed that the diameter of nodules,CT values and signal-to-noise ratio were not significantly different( P > 0. 05). Virtual unenhanced image detection of lymph node calcification was found in 86,with the detection rate of 93. 48%(86/92). The CT value of iodine images by virtual scanning and real scanning showed no significant difference( P > 0. 05); the enhanced strengthen value in virtual scanning was significantly higher than real scanning( P < 0. 05); CT valued of iodide images in virtual and real scanning were significantly higher enhanced strengthen value( P < 0. 01). The diagnostic sensitivity in CT value of iodine images was significantly higher than that in conventional scanning and virtual scanning( P < 0. 05); the diagnostic specificity in virtual scanning was significantly higher than that in CT value of iodide images and conventional scanning( P < 0. 05); the diagnostic accuracy in CT value of iodide images was slightly higher than conventional scanning and virtual scanning,but showed no significant difference( P > 0. 05). Conclusion Dual-source CT for virtual scanning has clinical value in differential diagnosis of benign and malignant solitary pulmonary nodules.
