【摘要】 目的观察水滤红外线A(w IRA)治疗慢性化脓性中耳炎(CSOM)的临床疗效。方法选取2010年1月至2017年1月该院耳鼻喉科收治的CSOM患者120例,采用随机数字表法将其分为A、B、C、D组,每组30例。A组为空白组,采用过氧化氢清洗外耳道,每天1次,连续应用15 d。B组采用过氧化氢清洗外耳道后利用w IRA局部辐照治疗,每天1次,每次20 min,连续应用5 d。C组采用过氧化氢清洗外耳道后利用w IRA局部辐照治疗,每天1次,每次20 min,连续应用10 d。D组采用过氧化氢清洗外耳道后利用w IRA局部辐照治疗,每天1次,每次20 min,连续应用15 d。记录各组患者治疗前后各症状疼痛视觉模拟评分(VAS)及纯音听阈、耳内镜、乳突CT检查变化情况,以评价疗效。结果 B、C、D组患者总有效率、各症状VAS,以及纯音听阈、鼓膜愈合率、乳突CT检查结果均明显优于A组,差异均有统计学意义(P<0.05);D组患者总有效率、各症状VAS,以及纯音听阈、鼓膜愈合率、乳突CT检查结果均明显优于B、C组,差异均有统计学意义(P<0.05)。结论 w IRA治疗CSOM是一种安全、有效的疗法。更多还原

【Abstract】 Objective To observe the clinical effect of water filtration infrared A(wIRA)in the treatment of chronicsuppurative otitis media(CSOM). Methods One hundred and twenty cases of CSOM in the department of otorhinolaryngologyhead and neck surgery,Second Affiliated Hospital of Chongqing Medical University from January 2010 to January 2017 wereselected and divided into the group A,B,C and D groups according to the random number table method,30 cases in each group.The group A was the blank group and adopted hydrogen peroxide for flushing external ear canal,once daily for continuous 15 d.The group B used wIRA local irradiation after flushing external ear canal by hydrogen peroxide,once daily,20 min each time forcontinuous 5 d. The group C used wIRA local irradiation after flushing external ear canal by hydrogen peroxide,once daily,20 min each time for continuous 10 d. The group D used wIRA local irradiation after flushing external ear canal by hydrogen peroxide,once daily,20 min each time for continuous 15 d. Then the changes of symptom and pain VAS scores,pure tone auditorythreshold,otoscopy and mastoid process CT examination before and after treatment were recorded in each group for assessing thecurative effect. Results The total effective rate,VAS score of each symptom,pure tone auditory threshold,tympanic membranehealing rate and mastoid process CT examination results in the group B,C and D were significantly better than those in the groupA,the differences were statistically significant(P<0.05);the total effective rate,VAS score of each symptom,pure tone auditorythreshold,tympanic membrane healing rate and mastoid process CT examination results in the group D were significantly betterthan those in the group B and C, the difference was statistically significant(P<0.05).
