伊曲康唑联合特比萘芬序贯疗法治疗甲真菌病有效性和安全性的Meta分析
Efficacy and security of sequential therapy with itraconazole and terbinafine pulse in the treatment of onychomycosis:a Meta-analysis
【摘要】 目的分析评价伊曲康唑联合特比萘芬序贯疗法治疗甲真菌病的有效性和安全性。方法计算机检索Pub Med、Cochrane library、EMBase、中国知网、万方、维普数据库中有关伊曲康唑联合特比萘芬序贯疗法治疗甲真菌病有效性和安全性的文献,对符合入选标准的文献进行质量评价,并采用Rev Man 5.3软件进行Meta分析。结果共纳入合格文献10篇,纳入患者总数为872例。Meta分析结果显示,与对照组(单纯伊曲康唑冲击疗法)相比,试验组(伊曲康唑联合特比萘芬序贯疗法)治疗甲真菌病的近期[比值比(OR)=3.24,95%可信区间(95%CI)2.17~4.83,P<0.01]及远期(OR=4.94,95%CI 2.63~9.29,P<0.01)总有效率均更高,复发率更低(OR=0.33,95%CI 0.21~0.54,P<0.01),差异均有统计学意义;而两组不良反应发生率比较,差异无统计学意义(OR=0.74,95%CI 0.44~1.24,P>0.05)。结论伊曲康唑联合特比萘芬序贯疗法治疗甲真菌病的有效性和安全性均较高。
【Abstract】 Objective To evaluate the efficacy and security of sequential therapy with itraconazole and terbinafine pulse in the treatment of onychomycosis. Methods Retrivaled from Pub Med,Cochrane Library,EMBase,CNKI,Wanfang,VIP database,literatures about efficacy and security of sequential therapy with itraconazole and terbinafine pulse in the treatment of onychomycosis were collected. Two reviewers independently evaluated the quality of the included literatures. The Rev Man 5.3 software was used for Meta-analysis. Results A total of 10 studies were included,involving 872 patients. Meta-analysis showed that compared with the control group(simple itraconazole pulse therapy),the experimental group(sequential therapy with itraconazole and terbinafine pulse)had higher recent [odds ratio(OR)=3.24,95% confidence interval(95%CI)2.17 to 4.83,P<0.01] and long-term(OR=4.94,95%CI 2.63 to 9.29,P<0.01)total effective rate and lower recurrence rate(OR=0.33,95%CI 0.21 to 0.54,P<0.01),the differences were statistically significant;but there was no significant difference in the rate of adverse reaction(OR=0.74,95%CI 0.44 to 1.24,P>0.05). Conclusion Sequential therapy with itraconazole and terbinafine pulse has favorable efficacy and security in the treatment of onychomycosis.
