血液恶性肿瘤患者血清脂蛋白a和胸苷激酶1的表达及临床意义

【摘要】 目的探讨血液恶性肿瘤患者血清脂蛋白a[LP(a)]和胸苷激酶1(TK1)水平的变化及其临床意义。方法选择2013年6月至2017年6月安阳市第五人民医院收治的血液恶性肿瘤患者109例(血液肿瘤组),其中急性非淋巴细胞白血病(ANLL)31例(ANLL组),急性淋巴细胞白血病(ALL)27例(ALL组),恶性淋巴瘤(ML)19例(ML组),多发性骨髓瘤(MM)17例(MM组),骨髓增生异常综合征(MDS)15例(MDS组)。选择同期体检健康者45例为对照组。对照组受试者于体检当日、血液恶性肿瘤患者于入院后次日及化学治疗缓解或化学治疗4个疗程后晨起空腹抽取肘静脉血,采用免疫比浊法检测血清LP(a)水平,采用化学发光法检测血清TK1水平,对血液肿瘤组、ANLL组、ALL组、ML组、MM组、MDS组及对照组受试者血清LP(a)和TK1水平进行比较,并对血液恶性肿瘤患者化学治疗前后血清LP(a)和TK1水平进行比较。结果血液肿瘤组、ANLL组、ALL组、ML组、MM组及MDS组患者血清LP(a)和TK1水平显著高于对照组(P<0.05),ANLL组、ALL组、ML组、MM组及MDS组患者血清LP(a)、TK1水平比较差异无统计学意义(F=2.319、4.783,P>0.05)。ANLL组、ALL组、ML组、MM组及MDS组患者化学治疗后血清LP(a)和TK1水平显著低于化学治疗前(P<0.05)。结论血液恶性肿瘤患者血清血清LP(a)和TK1水平升高,检测血清LP(a)和TK1水平对血液恶性肿瘤诊断及治疗效果评估具有一定的意义。

【Abstract】 Objective To investigate the changes and clinical significance of serum lipoprotein a [LP(a) ]and thymidine kinase 1(TK1) levels in patients with hematological malignancies.Methods A total of 109 patients with hematological malignancies in the Fifth People’s Hospital of Anyang City from June 2013 to June 2017 were selected as the subjects(hematological malignancies group).Among the patients,there were 31 cases of acute non lymphocytic leukemia(ANLL)(ANLL group),27 cases of acute lymphoblastic leukemia(ALL)(ALL group),19 cases of malignant lymphoma(ML)(ML group),17 cases of multiple myeloma(MM)(MM group) and 15 cases of myelodysplastic syndrome(MDS)(MDS group).A total of45 healthy subjects were selected as the control group at the same time.The elbow vein blood of the subjects in the control group(on the day of physical examination) and the patients in the hematological malignancies group(on the next day after admission,after 4 courses of chemotherapy or remission after chemotherapy) was collected in the fasting state.The serumLP(a)level was detected by immunoturbidimetry,and the serum TK1 level was detected by chemiluminescence method.The levels of serumLP(a) and TK1 in the hematological malignancies group,ANLL group,ALL group,ML group,MM group,MDS group and control group were compared.The levels of serumLP(a) and TK1 were compared before and after chemotherapy in patients with hematological malignancies.Results The levels of serumLP(a) and TK1 of the patients in hematological malignancies group,ANLL group,ALL group,ML group,MM group and MDS group were significantly higher than those in the control group(P<0.05).There was no significant difference in the levels of serumLP(a) and TK1 among the ANLL group,ALL group,ML group,MM group and MDS group(F=2.319,4.783;P>0.05).The levels of serumLP(a) and TK1 after chemotherapy were significantly lower than those before chemotherapy in the ANLL group,ALL group,ML group,MM group and MDS group(P<0.05).Conclusion The levels of serumLP(a) and TK1 increased in the patients with hematologic malignancies.The detection of serumLP(a) and TK1 levels has the certain significance for the diagnosis and the curative effect evaluation of hematological malignancies.
