HbA1c水平对急性心肌梗死患者PCI术后心脏功能与MACE发生率及死亡率的影响
Influence of HbA1c Levels on Cardiac Function,MACE Risk and Mortality of Acute Myocardial Infarction (AMI) Patients with PCI
【摘要】 目的探讨糖化血红蛋白(HbA1c)水平对行经皮冠状动脉介入治疗(PCI)术急性心肌梗死患者术后心脏功能、主要心血管不良事件(MACE)发生率及死亡率的影响。方法选取我院收治行PCI术急性心肌梗死患者100例,依据术前静脉HbA1c水平分为两组,其中A组患者HbA1c值<8%,而B组患者HbA1c值≥8%,每组各50例;比较两组患者术后Killip心功能分级、NYHA心功能分级、左室舒张末期内经(LVDd)水平、左心房大小(LAd)水平、左心室射血分数(LVEF)水平、随访MACE发生率及死亡率。结果 A组患者术后Killip心功能分级水平显著优于B组(P<0.05);A组患者术后NYHA心功能分级水平显著优于B组(P<0.05);A组患者LVDd、LAd及LVEF水平均显著优于B组(P<0.05);A组患者随访MACE总发生率显著低于B组(P<0.05);A组患者随访死亡率显著低于B组(P<0.05)。结论高HbA1c水平可能影响行PCI术急性心肌梗死患者术后心脏功能恢复,导致MACE和死亡风险上升。
【Abstract】 Objective To investigate the influence of HbA1 c levels on cardiac function,MACE risk and mortality of AMI patients with PCI. Methods 100 AMI patients with PCI were chosen in the period from May 2013 to December 2016 in our hospital and divided into group A( 50 patients) with vein HbA1 c before PCI < 8% and group B( 50 patients) with vein HbA1 c before PCI≥8%. The Killip cardiac function classification,NYHA cardiac function classification and the levels of LVDd,LA and LVEF after operation,the MACE incidence and mortality with follow-up of both groups were compared. Results The Killip cardiac function classification after operation of group A was significantly better than group B( P < 0. 05). The NYHA cardiac function classification of heart function after operation of group A was significantly better than group B( P < 0. 05). The levels of LVDd,LA andd LVEF of group A were significantly better than group B( P < 0. 05). The mortality with follow-up of group A were significantly lower than group B( P < 0. 05). The MACE incidence with follow-up of group A was significantly lower than group B( P < 0. 05). Conclusions High HbA1 c on AMI patients with PCI can seriously affect the recovery process of cardiac function after operation and cause to the increased MACE and mortality risk.
