【摘要】 目的分析甲状腺手术中甲状旁腺显露及功能保护的方法。方法回顾分析该院2015年1月—2018年4月期间接受甲状腺手术的56例患者临床资料,所有患者均行术中显露并保护甲状旁腺及血供的方法,观察术后患者的甲状旁腺功能。结果 56例患者均顺利完成手术,术中2例患者未找到明确的甲状旁腺。术中显露上甲状旁腺共计53枚,其中16枚确切显露上甲状旁腺血管;显露下甲状旁腺共计88枚,23枚确切显露下甲状旁腺血管。术后发现8例患者低钙血症,发生率为14.29%,其中24例行一侧叶全切除术、11例行一侧叶全切除术和对侧叶次全切除术、14例行甲状腺全切除术、7例行甲状腺全切+颈部淋巴结清扫术。结论行甲状腺手术时可在直视下显露和保护甲状旁腺,术中精细的解剖、保护甲状旁腺及其血供能够显著减少术后甲状旁腺功能减退,利于患者恢复。更多还原

【Abstract】 Objective To analyze the method of parathyroid exposure and functional protection in thyroid surgery.Methods Retrospective analysis of the clinical data of 56 patients who underwent thyroid surgery from January 2015 to April 2018 in the hospital. All patients underwent intraoperative exposure and protection of parathyroid glands and blood supply, parathyroid function. Results All 56 patients underwent surgery successfully. 2 patients did not find a clear parathyroid gland during the operation. During the operation, a total of 53 parathyroid glands were exposed, 16 of which revealed the parathyroid blood vessels. A total of 88 hypoparathyroid glands were revealed. 23 parathyroid blood vessels were revealed. Postoperatively, hypocalcemia was found in 8 patients with an incidence rate of 14.29%. Among them, 24 patients underwent total resection of the side lobe, 11 patients underwent one-sided total resection and subtotal resection, and 14 underwent thyroidectomy total resection, 7 cases of total thyroidectomy and cervical lymph node dissection. Conclusion During thyroid surgery, the parathyroid glands can be exposed and protected under direct vision. Fine intraoperative anatomy, protection of parathyroid glands and their blood supply can significantly reduce postoperative hypoparathyroidism and facilitate recovery.更多还原
