肺动脉闭锁合并动脉导管未闭的形态学分析
Morphological analysis to ductus arteriosus in patients with pulmonary atresia
【摘要】 目的:分析肺动脉闭锁(PA)患者中动脉导管未闭(PDA)的起源、形态及肺动脉发育情况。方法:回顾41例PA合并PDA患者的影像学资料结合胚胎学理论,分析PDA及肺动脉的形态学特点。结果:动脉导管连接肺动脉融合部35例,其中起源于主动脉弓降部32例,起源于锁骨下动无脉或无名动脉3例;双侧PDA例,导管分别起源于主动脉弓降部及无名动脉或锁骨下动脉根部,连接同侧肺门部肺动脉,无肺动脉融合部;动脉导管连接同侧肺门部肺动脉,另一侧肺由体肺侧枝供应肺血2例。结论:PA合并的PDA起源、形态及供血范围可出现较多变异;连接肺动脉融合部时动脉导管多合并不同程度的狭窄,连接一侧肺动脉时动脉导管多无明显狭窄甚至出现不同程度的脉动脉高压。
【Abstract】 Objective: To identify the morphological characteristics of patent ductus arteriosus( PDA)in patients with pulmonary atresia( PA). Methods: We reviewed imaging data of 41 patients with complex congenital heart disease featuring PA from June 2011 to December 2016. Results: 35 patients had the confluence of left and right pulmonary arteries,pulmonary blood flow was supplied through the PDA. PDA arises from the underside of aortic arch in 32 patients,PDA arises from brachiocephalic artery or subclavian artery in 3 patients. Pulmonary flow bilaterally provided by PDA in 4 patients. Pulmonary blood supplied through the PDA to one lung and through aortopulmonary collateral to the contralateral lung in 2 patients. Conclusion: The numerous of variation in origin,morphology and pulmonary blood supply of PDA in PA,when connected to confluence of pulmonary arteries,most of PDA had different degrees of stenosis,when connected to ipsilateral distal pulmonary arteries,most of PDA were widely patent and result in pulmonary hypertension.
