阴道微生态失衡对胎膜早破影响的调查研究

【摘要】 目的孕妇群体中常见有阴道微生态失调,可导致胎膜早破。本研究旨在分析胎膜早破产妇的阴道微生态特征。方法采用病例对照研究,采集研究对象下阴道2/3的白带样本进行阴道微生态检测,判定研究对象的阴道微生态状况。结果共纳入2 732名产妇进行阴道微生态的基本情况分析,其中胎膜早破者62名,正常分娩产妇2 670名,胎膜早破妇女的阴道微生态失衡状况高于正常组。影响产妇发生胎膜早破的阴道微生态失衡因素为乳酸杆菌比例失调(OR=2. 57,P <0. 001)、阴道出现炎症白细胞(OR=1. 75,P=0. 043)、革兰阳性球菌阳性(OR=2. 47,P=0. 004)、Nugent评分较高(OR=1. 95,P=0. 004)。结论本地区产妇阴道微生态存在失衡状况,临床医生应在产妇分娩前检测阴道微生态环境,必要时采取干预措施以减少妊娠不良结局的风险。

【Abstract】 Objective To characterize the vaginal microbiota of women following preterm premature rupture of membranes( PROM) since vaginal microbiome disorder is one of the most common conditions among pregnant women and associated with premature rupture of membranes. Methods A case-control study was conducted to collect vaginal samples of 2/3 low vagina for vaginal microecology detection,and the vaginal microecology of the study subjects was investigated. Results A total of2732 women in perinatal pregnant were enrolled. The dysbiosis of vaginal microecology in PROM group was higher than in non-PROM group. The vaginal microecological dysbiosis that affected perinatal pregnant women with PROM were decreased percentage of Lactobacillus( OR = 7. 36,P < 0. 001),positive for white blood cell( OR = 1. 75,P = 0. 043),positive for Gram-positive cocci( OR = 2. 47,P = 0. 004) and higher Nugent score( OR = 1. 95,P = 0. 004). Conclusion The dysbiosis of vaginal microecology exists in pregnant women with PROM in this region. Clinicians should test the vaginal microecological environment before delivery. Reasonable intervention measures should be proposed to reduce the risk of adverse pregnancy outcomes.
