【摘要】 目的探究江苏省昆山市1981-2015年胃癌所致的疾病负担和间接经济负担。方法 1981-2015年胃癌发病和死亡病例来源于肿瘤登记和死因监测,以伤残调整生命年(DALY)为疾病负担评价指标,以人力资本法计算因胃癌所致间接经济负担。结果 1981-2015年昆山市户籍居民因胃癌死亡8 617例,其中男性和女性分别死亡5 885例和2 732例。在1981-1992、1993-2005、2006-2015年3个时期,总人群中胃癌所致DALY分别为7.9/千人、7.2/千人和5.2/千人,因胃癌所致间接经济负担为8 305.6万元、61 924.9万元和155 705.9万元。2006-2015年期间,男性、女性及总人群中YLL占DALY的比例分别为78.9%、74.9%和77.5%。结论昆山市胃癌所致的DALY和间接经济负十分巨大;胃癌所致的疾病负担与间接经济负担在年龄之间并不对称,提示在人群中有效预防胃癌,降低胃癌负担显得十分紧迫,特别是40~59岁年龄组人群。更多还原

【Abstract】 Objective To examine the disability-adjusted life years( DALY) and an indirect economic burden due to stomach cancer in Kunshan city,Jiangsu province,1981-2015. Methods Stomach data were collected from cancer registry and death surveillance system of Kunshan city. Age-specific DALY due to stomach cancer was calculated,and the indirect economic burden was estimated by human capital approach. Results During 1981 to 2015,there were totally 8617 deaths due to stomach cancer,including 5885 male and 2732 female. The DALY due to stomach cancer were 7. 9,7. 2 and 5. 2 per 1000 person for both sex in the period of 1981-1992,1993-2005 and 2006-2015. Meanwhile,the indirect economic burden caused by stomach cancer was 83. 056,619. 249,and 1557. 059 million Chinese yuan in the three periods respectively. Among the period of 2006-2015,the proportion of years of life lost( YLL) to DALY among male,female and both sex were 78. 9%,74. 9%,and77. 5%,respectively. Conclusion The health burden and economic burden of stomach cancer are substantial in Kunshan.Moreover,the age-specific DALY and indirect economic burden are not exactly symmetrical,which suggests that effective control strategies to lower the cost are urgently needed,especially among those whose age are between 40 and 59 years.更多还原
