【摘要】 目的观察骨碎补联合多西环素局部应用治疗中重度慢性牙周炎患者疗效及对龈沟液内前列腺素E₂（PGE₂）、肿瘤坏死因子-α（TNF-α）水平的影响。方法选择中重度慢性牙周炎患者22例,采用同一患者口腔内病情相同牙齿的自身对照设计。将入选患牙随机分为试验组和对照组,分别给予自制复方多西环素凝胶（由骨碎补、多西环素组成）及盐酸米诺环素软膏治疗,于基线时及上药后41 d分别采集龈沟液测定PGE₂和TNF-α水平,并分别记录患者牙龈指数（GI）、牙周袋深度（PD）以及附着丧失程度（AL）,观察各项指标变化情况。结果治疗后,2组GI、PD、AL以及龈沟液内PGE₂、TNF-α水平均明显低于治疗前（P均<0.05）,且试验组TNF-α水平明显低于对照组（P<0.05）。结论骨碎补联合多西环素局部应用治疗中重度慢性牙周炎能明显改善牙周炎症状,显著降低龈沟液中TNF-α和PGE₂水平,效果优于盐酸米诺环素软膏。更多还原

【Abstract】 Objective It is to observe the curative effect of drynaria rhizome combined with doxycycline in the treatment of patients with moderate and severe chronic periodontitis,and its influence on the level of prostaglandin E₂（ PGE₂） and tumor necrosis factor-α（ TNF-α） in gingival crevicular fluid. Methods 22 patients with moderate or severe chronic periodontitis were selected. The self control design of the same tooth in the same patient with the same condition in the oral cavity was adopted. The selected teeth were randomly divided into the experimental group and the control group. Homemade compound doxycycline gel（ made from drynaria rhizome and doxycycline） and Minocycline Hydrochloride Ointment were given respectively. Gingival crevicular fluid was collected at baseline and at 41 days after administration to detect the levels of PGE₂ and TNF-α. The gingival index（ GI）,periodontal pocket depth（ PD） and attachment loss degree（ AL） were recorded respectively. The changes were observed before and after the indexes. Results After treatment,the levels of GI,PD,AL and PGE₂ and TNF-α in gingival crevicular fluid of the two groups were significantly lower than those before treatment（ all P < 0. 05）,the level of TNF-α in the experimental group was significantly lower than that in the control group（ P <0. 05）. Conclusion Local application of drynaria rhizome combined with doxycycline can significantly improve the symptoms of periodontitis in the treatment of moderate and severe chronic periodontitis,significantly reduce the levels of TNF-αand PGE₂ in gingival crevicular fluid,the effect is slightly better than that of Minocycline Hydrochloride Ointment.更多还原
