【摘要】 目的:探讨网格化管理在社区卫生服务中的意义。方法:南磨房社区卫生服务中心和5个下属社区卫生服务站自2016年5月开始实施社区卫生服务网格化管理,比较网格化管理实施前后居民健康档案管理指标、慢性病管理指标和筛检阳性率指标。结果:实施网格化管理后,居民健康档案核实率、高血压和糖尿病患者的规范管理率和筛检阳性率都得到显著提升(均P<0.001)。结论:网格化管理可以有效提高社区卫生服务工作质量,有助于发挥社区卫生服务机构"守门人"的作用。更多还原

【Abstract】 Objective:To explore the significance of grid management model for community health services.Methods:Grid management has been put into practice at community health service institutions in Nanmofang district since May 2011.The check rate of the residents’ health archive,the effect of chronic disease management and screening positive rate were compared between before and after the management was carried out.Results:Since the grid management model was adopted,the check rate of the residents’ health archive has increased a lot,the effect of chronic disease management improved greatly,and screening positive rate grew much higher.Conclusion Grid management can significantly improve the performance of community health services and help community health service institution to play the role of"gatekeeper".更多还原
