冠心病患者ADMA、eNOS和NO水平变化及与Gensini评分的相关性
Changes of Serum ADMA,eNOS and NO and the Correlation of Gensini Score in Coronary Heart Disease Patients
【摘要】 目的:分析冠心病患者血清不对称性二甲基精氨酸(ADMA)、内皮型一氧化氮合酶(eNOS)及一氧化氮(NO)水平变化及与Gensini评分的相关性。方法:206例心脏病住院患者依据冠状动脉造影结果分为对照组(冠脉狭窄≤50%,n=84)和冠心病组(冠脉狭窄>50%,n=122);再将冠心病组分为单支病变组(n=44)、双支病变组(n=43)和多支病变组(n=35)。分别采用酶联免疫吸附法和硝酸还原酶法测定各组血清ADMA、eNOS、NO水平,同时按照ACC/AHA指南对各组患者进行Gensini评分;分析各指标组间差异及ADMA、eNOS、NO水平与Gensini评分的相关性。结果:单支、双支、多支病变组的ADMA水平和Gensini积分均高于对照组,且随冠脉病变支数增加时依次升高;eNOS和NO水平则明显低于对照组,并于病变冠脉增加时依次降低,差异均有统计学意义(P<0.05或P<0.01)。相关分析表明冠心病患者血清ADMA水平与Gensini积分呈显著正相关(r=0.526,P<0.05);eNOS及NO水平与Gensini积分呈显著负相关(r=-0.435,-0.356,均P<0.05)。结论:ADMA、eNOS和NO水平检测对冠心病辅助诊断和防治监测有重要临床意义。
【Abstract】 Objective:To analyze the contents and changes of ADMA,eNOS and NO in serum with coronary heart disease(CHD)patients,and to study the correlation of Gensini score.Method:According to their severity of coronary artery lesion,all cases were divided into two groups:normal control group(84 cases)and CHD(122 cases).CHD group was subdivided into simple branch pathological group(44 cases),double branch pathological group(43 cases),Multi-branch pathological group(35 cases).All cases were tested Gensini score by ACC/AHA,the level of ADMA,eNOS were deteted by ELISA and NO was detected by nitrate reductase.To analyze their changes in four groups and to study the correlation of between ADMA,eNOS,NO and Gensini score.Results:The level of ADMA and Gensini score were increased gradually from normal group to multi-branch group.The level of ADMA had significant positive relationship to Gensini score(r=0.526).The level of eNOS and NO were decreased gradually from normal group to multi-branch group.The level of eNOS and NO had significant negative relationship to Gensini score(r=-0.435,r=-0.356,respectively)in the CHD patients(P<0.05).Conclusion:The level of ADMA,eNOS and NO can be used as important clinical values in the auxiliary diagnosis and prevention monitoring of the CHD.
