多层螺旋CT影像重建在上颌后牙区种植中的应用研究

【摘要】 目的分析多层螺旋CT影像重建在上颌后牙区种植的精度。方法采用螺旋CT对30例需要在上颌后牙区进行种植的患者进行术前术后扫描,将获得的术前CT数据进行图像重建后对需种植的上颌后牙区分析评价可用骨高度、宽度,骨密度,上颌窦黏膜等情况。将术后CT数据进行重建后将种植体长度与剩余骨高度之和与术前可用骨高度相比、上颌窦黏膜是否增厚等评价多层螺旋CT对种植术前术后评价的精确性。结果种植后种植体长度与剩余骨高度之和与术前测量的可用骨高度相比差异无统计学意义,术后上颌窦黏膜厚度差异无统计学意义,所有CT重建图像均能清晰地显示上颌骨形态、密质骨和松质骨、上颌窦黏膜厚度等解剖结构,并能精确地计算上颌后牙区可用的骨高度、宽度。结论利用多层螺旋CT辅助上颌后牙区种植可获得精确的术前术后评估,具有重要的临床意义。

【Abstract】 Objective To analyze the accuracy of multi-slice spiral CT reconstruction in maxillary posterior region.Methods 30 patients who needed implants in the maxillary posterior teeth were scanned by spiral CT before and after operation.The preoperative CT data were reconstructed to evaluate the bone height,width,bone mineral density and maxillary sinus mucosa.CT data were reconstructed to evaluate the accuracy of multi-slice spiral CT in the preoperative and postoperative evaluation of the implant length and the remaining bone height.Results There was no significant difference between the length of implant and the height of residual bone and the height of available bone before implantation.There was no significant difference in the thickness of maxillary sinus mucosa after implantation.The height and width of the maxillary posterior teeth were calculated.All reconstructed CT images could clearly show the morphology of maxilla,dense cancellous bone and the thickness of maxillary sinus mucosa.It can accurately calculate the available bone height and width in maxillary posterior teeth.Conclusion Multi-slice spiral CT assisted maxillary posterior teeth implantation can obtain accurate preoperative and postoperative evaluation,which has important clinical significance.
