不同固定方式对老年桡骨远端骨折骨代谢及关节功能恢复的影响分析

【摘要】 目的探讨不同固定方式对老年桡骨远端骨折骨代谢及关节功能恢复的影响。方法选取200例桡骨远端骨折老年患者作为研究对象,随机分为两组,外固定组予以手法复位夹板外固定治疗,内固定组予以切开复位钢板内固定治疗。对比两组治疗前后的关节功能及血清相关因子水平。结果两组治疗后的掌倾角、尺偏角、桡骨高度及血清TXB2、6-Keto-PGF1α、CTX-Ⅰ、RANKL、OPG水平差异有统计学意义,内固定组治疗后的掌倾角、尺偏角、桡骨高度均高于外固定组(P<0.05),内固定组治疗后的TXB2、CTX-Ⅰ、RANKL水平高于外固定组,6-Keto-PGF1α、OPG水平低于外固定组(P<0.05)。结论手法复位夹板外固定及切开复位钢板内固定在老年桡骨远端骨折的治疗方面各有优劣,外固定组虽然在解剖复位方面不及内固定组,但可有效改善骨代谢水平。

【Abstract】 Objective to investigate effect of different fixation methods on bone metabolism and joint function recovery of elderly patients with distal radius fracture. Methods choose 200 cases elderly patients with distal radius fracture as research objects, divide them into two groups randomly. External fixation group was treated with manual reduction and splint external fixation, and internal fixation group was treated with open reduction and steel plate internal fixation. Compare joint function and serum related factors level before and after treatment. Results after treatment, difference of palmar angle, ulnar angle, radial height and serum TXB2, 6-Keto-PGF1 alpha, CTX-I, RANKL and OPG levels showed statistical significance between two gropus. Palmar angle, ulnar angle and radial height of internal fixation group was higher than external fixation group(P< 0.05). TXB2, CTX-I and RANKL level of internal fixation group after treatment was higher than external fixation group, and 6-Keto-PGF1 and OPG level was lower than external fixation group(P< 0.05). Conclusion manual reduction splint external fixation and open reduction plate internal fixation has advantages and disadvantages in treatment of elderly patients with distal radius fracture. Anatomical reduction effect of external fixation group is less than internal fixation group, but it can improve bone metabolism level effectively.
