【摘要】 目的为研究急性肺损伤(ALI)患者血浆和支气管肺泡灌洗液(BALF)中脂联素(APN)、血栓素B2(TXB2)、6-酮-前列腺素F1α(6-keto-PGF1α)水平改变及与肺损伤水平的关系。方法随机抽取于2015年8月至2017年8月在我院接受治疗的60例ALI患者,另选同期在我院查体中心体检者60例作为对照组,采取放射免疫法测定血浆及BALF中TXB2、6-keto-PGF1α浓度,并记录下当时ALI患者的肺损伤评分。结果 ALI组血浆中TXB2浓度高于正常对照组(P<0.05),APN、6-keto-PGF1α浓度低于对照组(P<0.05),且血浆中APN差异显著(P<0.01);ALI患者血浆中APN与肺损伤评分呈负相关(r=0.86,r~2=0.75,P<0.01)。结论 ALI患者血浆和BALF中APN均下降,BALF中6-keto-PGF1α的表达也削减,血浆中APN改变更加明显,通过检测血浆中APN更有助于早期诊断ALI,并对ALI的严重程度有一定参考意义。更多还原

【Abstract】 Objective To study the relationship between the changes of APN、TXB2、6-keto-PGF1α in plasma and bronchoalveolar lavage fluid(BALF) in patients with acute lung injury and the level of lung injury. Methods At random,60 patients with acute lung injury who received treatment in our hospital from August 2015 to August 2017 were randomly selected. In addition, 60 cases of patients in the physical examination center of our hospital were selected as the control group in the same period, and radioimmunoassay was used to determine the degree of TXB2 and 6-keto-PGF1α in the plasma and bronchoalveolar lavage fluid, and the score of lung injury in patients with acute lung injury was recorded. Results The levels of TXB2 in the plasma of acute lung injury(ALI) group were higher than those of normal control group(P<0.05), the concentrations of adiponectin and 6-keto-PGF1α in serum and BALF were lower compared with those in normal control group(P<0.05), and significant difference in the levels of APN which are in plasma(P<0.01).The levels of TXB2 in BALF of ALI group are higher than those of normal control group,furthermore difference of TXB2 levels in BALF were significant(P<0.01); the APN levels in plasma of patients with acute lung injury were negatively correlated with the lung injury pathological score(r=0.86 r2=0.75 P<0.01). Conclusion The adiponectin levels in plasma and BALF all increased in the patients who were suffered with acute lung injury,6-keto-PGF1α levels in BALF expression also decreased,the decrease in plasma APN levels changes were more significant. So the detection APN levels in plasma maybe help more in the early diagnosis of Acute Lung Injury, and they may have certain reference significance to analyze the severity of acute lung injury.更多还原
