链球菌感染与儿童过敏性紫癜相关性研究

【摘要】 目的探讨链球菌感染在儿童过敏性紫癜(HSP)及紫癜性肾炎(HSPN)发生发展中的作用,并分析抗链球菌溶血素O(ASO)与血WBC、CRP、血浆白蛋白、球蛋白、血肌酐、Ig A、Ig G、Ig M、Ig E、补体C3、C4、尿蛋白、尿潜血之间的相关性。方法将154例确诊HSP的住院患儿根据ASO滴度分为两组(ASO阳性组与ASO阴性组),记录患儿发病情况及相关实验室指标,分析两组数据之间数据差异,并对ASO与其他数据进行相关性分析。结果 HSP患儿中存在链球菌感染者占比33.8%;ASO阳性组患儿年龄、血清球蛋白、肌酐、Ig A值均较ASO阴性组高,差异具有统计学意义;且在HSP中ASO值与上述指标存在相关性;ASO阳性组患儿较ASO阴性组患儿更容易出现尿潜血异常,两组之间尿蛋白差异并不明显。ASO阳性组患儿与ASO阴性组患儿中肾脏受累比例卡方分析后,P>0.05,差异不具有统计学意义。结论链球菌感染与儿童HSP发病有关,但对HSPN的发生关系不明显;ASO滴度与患儿年龄、肌酐、球蛋白、Ig A、Ig G存在相关性;存在链球菌感染的HSP患儿更易出现尿潜血异常。

【Abstract】 Objective To investigate the role of streptococcal infection in the development of children with Henoch-Schonlein purpura(HSP) and Henoch-Schonlein purpura nephritis(HSPN), and analyze correlations between antistreptolysin O(ASO) and blood WBC, CRP, plasma albumin, globulin, serum creatinine, Ig A, Ig G, Ig M, Ig E, complement C3, complement C4, urinary protein, and urine occult blood. Methods A total of 154 hospitalized children with HSP were divided into two groups(ASO positive group and ASO negative group) according to the ASO titer. The General situation of the patients and laboratory indicators were recorded. Then analysis the data differences between the two groups. Analysis the correlation between ASO and other data. Results The proportion of streptococcal infection in children with HSP was 33.8%. The age, serum globulin, creatinine and Ig A values in ASO positive group were higher than those in ASO negative group. The differences was statistically significant; There was a significant correlation between the ASO titer and the indicators which were described above; Children with ASO positive group were more prone to urine occult blood abnormalities than those with ASO negative group, and there was no significant difference in urinary protein between the two groups. The proportion of kidney involvement in the ASO positive group and the ASO negative group was proportional to the Chi-square analysis, P>0.05, and the difference was not statistically significant. Conclusion Streptococcal infection is related to the incidence of HSP in children, but the relationship between streptococcal infection with HSPN is not obvious; ASO titer is correlated with age, creatinine, globulin, Ig A and Ig G in children; HSP with streptococcal infection are more likely to have urine occult blood abnormalities in children.
